Integrating Screening and Treatment of Unhealthy Alcohol Use and Depression with Screening and Treatment of Anxiety, Pain, and Other Substance Use Among People with HIV and Other High-Risk Persons

Abstract: We review and synthesize results from a series of analyses estimating the benefit of screening for unhealthy alcohol use, depression, and tobacco to detect individuals at heightened risk for co-occurring anxiety, pain, depression, unhealthy alcohol use, and other substance use among people with HIV and HIV-uninfected individuals in the Veterans Aging Cohort Study. We also examine the potential impact of reducing unhealthy alcohol use and depressive symptoms on the incidence of co-occurring conditions. We found… Show more

“…Caniglia et al proposed that screening and treatment for alcohol and depression may help alleviate comorbidity of anxiety, other substance abuse, etc. in PLWH ( Caniglia et al, 2021 ). It’s time to incorporate screening and detox for alcohol, drugs, and cocaine into routine clinical care for HIV, especially the management of alcohol in men ( Lancaster et al, 2021 ; Maisto et al, 2021 ).…”

“…Finally, we simulated the influence of screening for alcohol, depression, and tobacco and conducting a comprehensive diagnostic assessment and treatment of all psychiatric and substance use conditions (depression, anxiety, pain, and alcohol, marijuana, illicit opioid, and stimulant use), given the strong clustering of these conditions. 18 To assess LE gains, we compared each screening/ decarceration scenario to a counterfactual assuming 100% of cohort members would be exposed to incarceration, and no additional CASM screening would occur. LE differences between each intervention scenario and the counterfactual arose from the following modeled effects: health benefits from improvements in CASM conditions due to decarceration and health benefits from improvements in CASM conditions that were treated.…”

“…Adrawa et al also showed an association between alcohol use and non-adherence in community-based ART distribution settings in Uganda, with participants attributing forgetfulness as a major cause of nonadherence (47). Available data suggest that addressing mental health problems by early screening and interventions at the time of counseling, testing, ART initiation, and follow-up care may be critical for better outcomes for PWH (48, 49, 50).…”
