Management of chronic migraine is complex, and many patients are relatively refractory to therapy. Specialist referral will often be required and should not be unduly delayed. On the other hand, the primary care physician should be able to make the diagnosis, initiate therapy, and manage some less refractory patients without referral. The timing of referral should depend both on the expertise of the primary care physician in headache management and the patient's response to initial therapy.

ZUSAMMENFASSUNGDie Therapie akuter Migräneattacken und die Prophylaxe der Migräne wurde durch Fortschritte in den Erkenntnissen der Pathophysiologie der Migräne und die Entwicklung neuer Substanzklassen in den letzten 30 Jahren dramatisch verbessert. Dazu gehört die Einführung der Triptane. Antagonisten gegen Substanz-P waren allerdings nicht wirksam. In der Migräneprophylaxe waren neben den etablierten Medikamenten auch Topiramat und bei der chronischen Migräne Onabotulinumtoxin A wirksam. Neue Ansätze ergeben sich durch die neuen monoklonalen Antikörper gegen CGRP oder den CGRP-Rezeptor. Wichtige Fortschritte gab es in der Epidemiologie und Therapie des chronischen Kopfschmerzes durch Übergebrauch von Schmerz- oder Migränemittel. Patienten mit diesem Krankheitsbild sollten bevorzugt in Schwerpunktpraxen oder Kopfschmerzzentren behandelt werden.

A growing amount of evidence suggests that magnesium deficiency may play an important role in several clinical conditions concerning women health such as premenstrual syndrome, dysmenorrhea, and postmenopausal symptoms. A number of studies highlighted a positive correlation between magnesium administration and relief or prevention of these symptoms, thus suggesting that magnesium supplementation may represent a viable treatment for these conditions. Despite this amount of evidence describing the efficacy of magnesium, few and un-systematize data are available about the pharmacological mechanism of this ion for these conditions. Herein, we review and systematize the available evidence about the use of oral magnesium supplementation in several gynecological conditions and discuss the pharmacological mechanisms that characterize these interventions. The picture that emerges indicates that magnesium supplementation is effective in the prevention of dysmenorrhea, premenstrual syndrome, and menstrual migraine and in the prevention of climacteric symptoms.
