Infusible platelet membranes improve hemostasis in thrombocytopenic blood: experimental studies under flow conditions

Abstract: IPM promotes local procoagulant activity at sites of vascular damage under conditions of severe and moderate thrombocytopenia. IPM also appears to facilitate platelet cohesive functions under conditions of moderate thrombocytopenia.

“…Such platelet microparticles form spontaneously during the storage of platelets and have been found in platelet concentrates, fresh-frozen plasma, and cryoprecipitate. These microparticles, like intact platelets, possess procoagulant activity; adhere to the vascular subendothelium; and enhance platelet adhesion [19,20]. Taking into account the functionality of platelet microparticles and their hemostatic properties similar to those of intact platelets, it was reasonable for investigators to examine the potential of microparticles as a possible platelet substitute.…”

“…This company's infusible platelet membranes (IPM) were prepared from outdated platelet concentrates. This IPM, which consisted of spherical vesicles with a diameter of approximately 0.6 lm, has been shown to contain various procoagulant phospholipids [19].…”

“…Unfortunately, development of the product has apparently not been continued. An alternative strategy is to better understand the role of platelet phospholipids in hemostasis and potentially use them as a platelet substitute 12 . Although a very large amount of work remains, the fact that an artificial platelet product was in clinical trial is a remarkable accomplishment.…”

The potential impact of a platelet substitute on transfusion therapy and transfusion medicine is substantial.…”

“…IPM have been shown in vivo to shorten the prolonged ear bleeding time in thrombocytopenic rabbits for at least 6 h postinfusion. However, by 24 h, the hemostatic activity is no longer detectable (Galán et al. , 2000).…”
