Magnesium ammonium phosphate hexahydrate, called struvite, is the dominant component of infectious urinary stones. In addition to struvite, infectious urinary stones include solid phases with poor crystallinity as well as amorphous matter. This article is devoted to the analysis of these solid phases, because they have not been characterized well until now. The solid phases tested were obtained from artificial urine in the absence and presence of Proteus mirabilis. The solid phases were characterized by different techniques (X-ray Diffraction, Energy Dispersive X-ray, Scanning Electron Microscopy, as well as Raman and Infrared Spectroscopies). According to the results these phases are carbonate apatite (CA), hydroxylapatite (HAP), amorphous calcium carbonate (ACC), amorphous calcium phosphate (ACP) and/or amorphous carbonated calcium phosphate (ACCP). Carbonate apatite and hydroxylapatite may occur in non-stoichiometric forms, i.e., various anions can be substituted for CO 3 2− , OH − , and PO 4 3− groups in them. The non-stoichiometry of carbonate apatite and hydroxylapatite also implies a deficiency of calcium ions, i.e., calcium ions may be partially replaced by other cations. Experimental techniques and chemical speciation analysis demonstrate that the presence of magnesium influences the formation of CA and HAP.

Abstract:The property changes of urinary nanocrystallites in 13 patients with calcium oxalate (CaOx) stones were studied before and after ingestion of potassium citrate (K 3 cit), a therapeutic drug for stones. The analytical techniques included nanoparticle size analysis, transmission electron microscopy, X-ray diffraction, and Fourier-transform infrared spectroscopy. The studied properties included the components, morphologies, zeta potentials, particle size distributions, light intensity autocorrelation curves, and polydispersity indices (PDIs) of the nanocrystallites. The main components of the urinary nanocrystallites before K 3 cit intake included uric acid, β-calcium phosphate, and calcium oxalate monohydrate. After K 3 cit intake, the quantities, species, and percentages of aggregated crystals decreased, whereas the percentages of monosodium urate and calcium oxalate dehydrate increased, and some crystallites became blunt. Moreover, the urinary pH increased from 5.96 ± 0.43 to 6.46 ± 0.50, the crystallite size decreased from 524 ± 320 nm to 354 ± 173 nm, and the zeta potential decreased from −4.85 ± 2.87 mV to −8.77 ± 3.03 mV. The autocorrelation curves became smooth, the decay time decreased from 11.4 ± 3.2 ms to 4.3 ± 1.7 ms, and the PDI decreased from 0.67 ± 0.14 to 0.53 ± 0.19. These changes helped inhibit CaOx calculus formation.

Biomineralization of struvite crystals by Proteus mirabilis from artificial urine and their mesoscopic structure

In order to investigate the mineralization of struvite we performed the experiment of struvite growth process from artificial urine. The crystallization process was induced by Proteus mirabilis, as these bacteria are mainly isolated from infectious stones. The crystallization process occurred at conditions mimicking the real urinary tract infection. Our results show that struvite exhibits polar properties. This feature of struvite crystals is potentially very important in the case of binding additives which may either enhance or inhibit crystallization process. It seems also that the differences in the polarity of opposite faces of c-axis play important role in directing the struvite mesoscopic arrangement. We also described recent developments concerning curcumin -pigment extracted from the roots of turmeric commonly known as a spice added to various food preparations. Curcumin exhibited the effect against Proteus mirabilis inhibiting the activity of urease and consequently decreasing the efficiency of struvite growth. Therefore, curcumin belongs to phytoterapheutic components, which may be the alternative with relation to the antibiotic therapy. The paper concludes with a future outlook and goals in this field of research.
