Articulando perspectivas teóricas para analisar a informática em saúde no Brasil Linking theoretical perspectives to analyze health informatics in Brazil ResumoAo longo dos últimos trinta anos, o Brasil vem desenvolvendo sistemas de informação em saúde (SIS). Porém, ainda hoje, esses SIS são fragmentados e tentativas de integrá-los fracassaram. A situação atual leva a questionamentos tais como: quais as barreiras existentes à utilização das tecnologias da informação e comunicação (TIC) na saúde? Quais são as consequências causadas à sociedade brasileira por tal situação? Em linha com isso, este ensaio articula duas escolas de pensamento -(a) TIC em países em desenvolvimento e (b) informação e informática em saúde -, de forma a propor um modelo para analisar a informatização da saúde no Brasil. Tal modelo considera a informática em saúde como uma política pública que objetiva desenvolver e aplicar TIC para atender às necessidades de saúde da sociedade, definindo atributos de qualidade para tal política, quais sejam: democratização, eficácia, preservação e sinergia. Além disso, o modelo permite analisar a trajetória dessa política pública por meio dos atores a ela associados -políticos, burocratas e sociedade civil -, os quais interagem em espaços influenciados por elementos como sistemas e padrões prévios, bem como com outras políticas públicas relacionadas. Health Information Systems and Democracy: Contributions from the Brazilian Sanitary Movement

Within the context of studies on Information and Communication Technologies for Development (ICT4D), earlier research has sought to understand how Health Information Systems (HIS) can achieve success in developing countries. In Brazil, in addition to these studies, research under the guise of Information and Information Technology in Health (IITH), developed within the context of the Brazilian Sanitary Movement, has also investigated how HIS can contribute to the development of health practices. IITH research seeks to understand how Health Information and Communication Technology (ICT) can enable the development of a democratic arena to improve health practices management in Brazil. This paper reviews the IITH literature in order to link its proposals with studies of HIS developed within the ICT4D realm. Thus, it was perceived that both theoretical approaches can be compared under six dimensions of analysis, namely: a) the need for a political approach to HIS analysis; b) the importance of integration standards for health information; c) the quest for research methods based on practical research; d) the need for a holistic approach to understand HIS implementation and use; e) the importance of local context settings; and f) the influence of technical artifacts on HIS. For each dimension, IITH and ICT4D contributions are compared and it is noted how similar they are in reality, as well as how they can learn and benefit from each other.

Participation in Discussion Spaces of Health Informatics in Brazil

Abstract. This work analyses instances of discussion related to the Brazilian public policy on health information and communication technologies (ICT).Departing from a theoretical model, which relates health ICT outcomes to instances of discussion, this work aims to identify how multiple actors participate on decision-making processes regarding health ICT. Laws and norms related to discussion forums about Brazilian health system were researched. The work shows that the discourse of participation has been present since SUS (Brazilian Unified Health System) beginnings, and, in recent years, new forums have been created. Thus, this paper concludes with the importance of a coordinated work between the forums, as well as the very need of participation of multiple actors in the instances of discussions about health ICT.Keywords: Health Information Systems, Public Policy, Informatics in Brazil's Health System. IntroductionOver the last twenty years, countries have bet on national strategies to incorporate information and communication technologies (ICT) into health practices [1][2][3][4][5] and scientific literature about Health Information Systems (HIS) reports that the informatization of health is a long-term process influenced by diverse actors [1,[6][7][8]. This is also the case in Brazil, where the first HIS emerged in the 1970s [9] and, after three decades, diverse advances have taken place in an effort to develop ICT for the benefit of health. In Brazil, the HIS were developed by actors in both public and private sectors to meet the demands of health planning and management, as was the elaboration of a national strategic vision of heath informatics [4]. However, recent attempts by the Ministry of Health (MoH) aiming to construct an Electronic Health Record (EHR) on a national level failed and the HIS produced in Brazil are fragmented, limiting the use of the information for the benefit of the Brazilian population [8].Considering the complex scenario related to the informatization of health in the Brazilian setting, it becomes necessary to study the factors associated with this process. Therefore, based upon the premise that participative actions can bring about better results for health ICTs, this work seeks to analyze the participation of diverse actors involved in the instances of discussions of health informatics in Brazil.
