Influence of proton pump inhibitors on gastritis diagnosis and pathologic gastric changes

Abstract: AIM:To investigate the influence of proton pump inhibitors (PPIs) exposure on the diagnosis of Helicobacter pylori (H. pylori) gastritis and intestinal metaplasia. METHODS:Chronic PPI use is associated with masking of H. pylori infection. Patients with H. pylori infection are predisposed to gastric and duodenal ulcers, and long-term infection with this organism has been associated with gastric mucosal atrophy and serious long-term complications, such as gastric lymphoma and adenocarcinoma. Three hundred patien… Show more

“…The most frequent reason for these atypical gastritis is the consumption of PPI or previous eradication treatment . PPI were introduced 25 years ago and changed the paradigm of H. pylori gastritis promoting the diagnosis of apparently H. pylori negative inactive chronic gastritis . The concept of the utility of ancillary stains for H. pylori in atypical gastritis was also proposed in another study …”

“…This wide variation is probably due not only to different population characteristics but mainly to different criteria used for the diagnosis of chronic gastritis. Nonetheless, the diagnosis of chronic gastritis may be significant in some populations, as it may suggest the possibility of H. pylori infection that is masked by the use of proton pump inhibitors [ 16 ]. The rate of intestinal metaplasia is also variable among studies but does not exceed 3.1% [ 3 , 5 , 7 , 8 , 11 , 12 ].…”

“…PCR is more sensitive as the grade of atrophy and/or IM progress toward malignancy[48]. Moreover, the accuracy of histopathology identification of H. pylori is affected by PPI and antibiotic use, which were not an exclusion criteria for these studies[49,50].…”
