Introduction: Third stage of labour start after delivery of baby and continues till delivery of placental and membranes. Post-partum hemorrhage is most common complication of this stage. Objectives: To compare the mean duration of third stage of labour in patients undergoing spontaneous vaginal delivery with and without placental cord blood drainage. Materials and Methods: This randomized control trial was carried out in three tertiary care hospitals of Punjab, Department of Obstetrics &Gynecology, Allied Hospital, Faisalabad, Lady Aitcheson Hospital Lahore and Lady Willington Hospital Lahore. From 1st July 2018 to 30th December 2018, 188 cases (94 in each group) were selected using non probability consecutive sampling. Total 188 patients were enrolled in the study after taking informed consent and allocated into control and case group by lottery method. In control group cord was clamped from both sides while in case group cord was unclamped from other side and duration of third stage was noted for both groups. Ethical approval was taken from hospital ethical committee. Results: The mean age was 29.06 ± 4.9 years with minimum age of 21 years and maximum age of 45 years. More patients belong to younger age group (20 years to 35 years) i.e. 119 while 69 belonged to elder age group i.e. 36 year to 50 years 63.2 % and 36.7% respectively. Mean duration of third stage of labour in case group was 8.5±2.9 minutes and in control group 10.8±5.4 minutes. More distribution of patients in younger age group 20 -35 years and less in 36 to 50 years i.e. 67% and 33% respectively. Conclusion: This study concluded that unclamping placenta from mother side significantly reduces the third stage of labour

Observational studies have demonstrated that placental cord drainage can shorten the length of the third stage of labour and reduce blood loss during vaginal deliveries. The aim of our work was to evaluate the existing evidence for the effectiveness of placental cord drainage in the third stage of labour. PubMed, Embase, the Cochrane Library, Web of Science, Google Scholar and 50 journals were searched up to the 4th of June, 2017. Randomized controlled trials comparing placental cord drainage with no cord drainage in the third stage of labour during vaginal delivery were included. Nine studies with 2653 participants were included. Compared with clamping the umbilical cord, umbilical cord drainage during the third stage of labour shortened the third-stage duration by 2.28 minutes (95% confidence interval (CI), −3.22 to −1.33), but did not reduce the amount of blood loss (−31.99 mL, −86.08 to 22.09). For women with normal vaginal deliveries, the incidence of postpartum haemorrhage was reduced by 3%. Placental cord drainage is a simple and non-invasive procedure that should be considered after delayed cord clamping. Further studies about the physiological processes and effects of placental cord drainage in additional circumstances are needed.

Acupuncture use in obstetrics has been increasing in Western medicine, especially to alleviate complications of pregnancy, the most important cause of maternal mortality worldwide. One quarter of maternal deaths are caused by complications in the third stage of labor, an interval between complete delivery of the baby and the complete expulsion of the placenta. This review analyzes the increased acupuncture use for pregnancy complications as a potential method of reducting maternal mortality during the third stage of labor. This overview focuses on acupuncture points and techniques to manage the third stage of labor and help patients with retained placentas. Passive (or expectant) management of the third stage of labor is occasionally associated with massive obstetric hemorrhage, a major cause of maternal morbidity and mortality, especially in low-income countries. Active management of the third stage of labor has been shown to reduce the risk of postpartum hemorrhage. Use of acupuncture in the first and second stages of labor could lead to a faster separation of the placenta in the third stage of labor. The possible effects of acupuncture in cases of retained placentas may have significant implications for possible complications and final outcomes of labor. Further studies are needed for more conclusive results.
