Chronic renal disease (CRD) is a pathophysiologic process with multiple etiologies, resulting in the inexorable attrition of Nephron number and function and frequently leading to end-stage renal disease (ESRD). In turn, ESRD represents a clinical state or condition in which there has been an irreversible loss of endogenous renal function, of a degree sufficient to render the patient permanently dependent upon renal replacement therapy (dialysis of transplantation) in order to avoid life threatening uremia, reflecting a dysfunction of all organ systems as a result of untreated or under treated acute or chronic renal failure. The current study was involved 80 patients, the age range within 25-70 years, selected sample of patients who attend Iraqi center of kidney dialysis, Baghdad Teaching Hospital and Al-Yarmok Teaching Hospital. All the patient's body mass index [BMI] were measured serum anti diuretic hormone ADH was assessed using enzyme linked immunosorbent kit [Elisa] while urea, creatinine, albumin, hemoglobin were determined by spectrophotometer (PD-303), and sodium, potassium, chloride were determined by Electrolyte analyzer 9180.These patients were divided in two group B (40 patients) as pre dialysis with ESRD, and group A (40) healthy control. Results showed non-significant elevations in ADH levels in the patient group compared with the control group, while highly significant increase in urea, creatinine and albumin levels in in the patient group compared with the control group, also highly significant decrease in hemoglobin comparing with the control group. Also, it was observed non-significant decrease in sodium, non-significant increase in potassium and highly significant increase in chloride when comparing with the control group.

Chronic renal failure (CRF) is one of the major public health problems in Iraq. This work aimed to investigate whether adenosine deaminase (ADA) activity has any correlation with calcium concentration in patients with CRF for the first time, then using this correlation as a diagnostic tool for CRF disease. Creatinine (Cr), urea, total protein concentrations, in addition to ADA activity and calcium concentration, were measured for 33 males individuals divided to two groups, the first group included eighteen patients with CRF before hemodialysis at department of artificial kidney of Al-Hussein Teaching Hospital in Thi-Qar (Iraq) while control group included fifteen healthy matched individuals. The results showed that a significant increase in Cr, urea concentrations in addition to ADA activity, while there was a significant decrease in a total protein and calcium concentrations for patients group comprised with control group (p < 0.05). Indeed, there was a weak negative correlation between ADA activity and calcium concentration according to this study.

Objective. We tested the hypothesis that postpartum combined oral contraceptive (COC) treatment would induce oxidative stress via the adenosine deaminase-xanthine oxidase pathway in the kidney. We also sought to determine whether mineralocorticoid receptor (MR) or glucocorticoid receptor (GR ) blockade would suppress the activities of ADA and xanthine oxidase caused by postpartum COC treatment in the kidney. Methods. Twenty-four Wistar dams were randomly assigned to 4 groups ( n = 6 / group ). Dams received vehicle (po), COC (1.0 μg ethinylestradiol and 5.0 μg levonorgestrel; po), COC with GR blockade (mifepristone; 80.0 mg/kg; po), and COC with MR blockade (spironolactone; 0.25 mg/kg; po) daily between 3rd and 11th week postpartum. Results. Data showed that postpartum COC caused increased plasma creatinine and urea, increased renal triglyceride/high-density lipoprotein ratio, free fatty acid accumulation, alanine aminotransferase, gamma-glutamyltransferase, uric acid, and activities of renal XO and ADA. On the other hand, postpartum COC resulted in decreased plasma albumin, renal glutathione, and Na+-K+-ATPase activity with no effect on lactate production. However, MR or GR blockade ameliorated the alterations induced by postpartum COC treatment. The present results demonstrate that MR or GR blockade ameliorates postpartum COC-induced increased activities of ADA and xanthine oxidase and restores glutathione-dependent antioxidative defense. Conclusion. These findings implicate the involvements of GR and MR in renal dysfunctions caused by COC in dams via disrupted glutathione antioxidative barrier.
