Influence of androgen deprivation therapy on choline PET/CT in recurrent prostate cancer

Abstract: We conclude that in order to prevent false-negative choline PET/CT scans androgen deprivation should be withheld prior to scanning, especially in hormone-naïve patients.

“…Consequently, the preferable imaging quality and clear visualisation resulted in questions about therapy response especially in patients with metastatic PCa during androgen deprivation therapy (ADT). During the last decades, choline analogues labelled with 11 C and 18 F were the ‘gold-standard’ in nuclear medicine PCa imaging [ 18 , 19 ]. Furthermore, they were used for staging and therapy monitoring.…”

“…There are limited studies which have evaluated this effect. It is recommended that androgen deprivation therapy should be put on hold prior to choline PET-CT to avoid false negative outcome (27). This is of utmost importance in clinical scenario of biochemical relapse of prostate cancer in hormone naïve patients.…”

“…The mechanisms of choline uptake and the role of the androgen receptor in prostate cancer treatment are most relevant when considering choline PET/CT under androgen deprivation. Dost et al stated that the current working hypothesis suggesting that androgen deprivation therapy does not influence choline PET/CT has not been overruled [4]. However, one should keep in mind that related clinical studies are limited in number and were not primarily designed to prospectively assess the effect of androgen deprivation on choline uptake of prostate cancer.…”
