Inflammatory pseudotumours of the paranasal sinuses

Abstract: Inflammatory pseudotumors are clinocopathologically distinctive but biologically controversial entities. They have been described in the lungs, abdomen, retroperitoneum and extremities, but rarely affect the head and neck region. This report is of 2 cases of pseudo tumor of the paranasal sinuses with varied clinical presentation and successful outcome following treatment.

“…Inflammatory pseudotumours of the lymph nodes characteristically demonstrate a fibro-inflammatory process in the tissues, 4 which is reflected in the case we have presented. Despite this, there are very few reports in which an inflammatory pseudotumour has developed in the presence of HIV positivity 5 7 . In two of the cases reported previously, the paranasal sinuses were identified as the site of development 6 , 7 …”

“…However, various stimuli may act as triggers for its development, such as unrecognized microorganisms, minor trauma, smoking and chronic irritation caused by cocaine abuse. 9,10 It may represent a chronic response or a delayed presentation related to remote or undetected trauma. 11 It has been postulated that they might be the result of a postinflammatory repair process, a metabolic disturbance or an antigen-antibody interaction with an agent that was no longer identifiable in aspiration or biopsy material.…”

“…8 Immunochemistry confirms the myofibroblastic phenotype of spindle cells. 9 Inflammatory pseudotumor is usually diagnosed by exemption since clinical and histopathologic findings are sometimes vague and inconsistent. It is generally accepted that a definite cytological diagnosis of inflammatory pseudotumor cannot be made by fine needle aspiration cytology because the predominant cell pattern is nonspecific.…”
