Envenomation or poisoning by toxins from animals poses an important health hazard in the tropics. Animal toxins are complex mixtures of proteins, peptides, enzymes and chemicals. These toxins exert their effects through modulation of ion channels and receptors, and via direct enzyme action. Depolarization or hyperpolarization of ion channels--caused by most marine toxins, and some snake and insect venoms--results in neuromuscular symptoms that can be associated with hemodynamic changes. Toxin enzymes, especially proteases and phospholipase A2, initiate inflammatory processes that involve the generation of proinflammatory cytokines and vasoactive mediators, resulting in systemic and renal hemodynamic alterations. Toxin enzymes also have direct effects on erythrocytes, myocytes, blood coagulation factors, vascular endothelium and epithelial cells. As a result, disseminated intravascular coagulation, bleeding diathesis, intravascular hemolysis and rhabdomyolysis are common after exposure to animal toxins. The renal manifestations of animal toxin envenomation, which are usually acute, result mainly from these enzymatic effects. All renal structures can be affected by animal toxins, and tubular necrosis is common. Acute kidney injury is attributed to decreased renal blood flow (associated with intravascular hemolysis or rhabdomyolysis), disseminated intravascular coagulation or direct tubular toxicity. Immunologic mechanisms have a minor role in the pathophysiology of nephropathy caused by animal toxins.

Snake venom metalloproteinases (SVMPs) affect the extracellular matrix (ECM) in multiple and complex ways. Previously, the combination of various methodological platforms, including electron microscopy, histochemistry, immunohistochemistry, and Western blot, has allowed a partial understanding of such complex pathology. In recent years, the proteomics analysis of exudates collected in the vicinity of tissues affected by SVMPs has provided novel and exciting information on SVMP-induced ECM alterations. The presence of fragments of an array of ECM proteins, including those of the basement membrane, has revealed a complex pathological scenario caused by the direct action of SVMPs. In addition, the time-course analysis of these changes has underscored that degradation of some fibrillar collagens is likely to depend on the action of endogenous proteinases, such as matrix metalloproteinases (MMPs), synthesized as a consequence of the inflammatory process. The action of SVMPs on the ECM also results in the release of ECM-derived biologically-active peptides that exert diverse actions in the tissue, some of which might be associated with reparative events or with further tissue damage. The study of the effects of SVMP on the ECM is an open field of research which may bring a renewed understanding of snake venom-induced pathology.

Tese apresentada ao Programa de Pós-Graduação Interunidades em Biotecnologia USP/Instituto Butantan/ IPT, para a obtenção do Título de Doutor em Biotecnologia. As metaloproteases presentes nos venenos ofídicos (SVMPs) são toxinas-chave envolvidas na patogênese do envenenamento, causando hemorragia e inflamação. A jararagina é uma metaloprotease hemorrágica da classe PIII isolada a partir do veneno de Bothrops jararaca, que em modelo experimental animal possui um efeito pró-inflamatório caracterizado principalmente por edema, liberação de citocinas e migração celular. Compreender como é a resposta inflamatória desencadeada por SVMPs, estabelecendo os principais mediadores inflamatórios no desenvolvimento do dano tecidual, pode contribuir de forma significativa com o tratamento dos efeitos locais dos envenenamentos ofídicos. Neste estudo nós avaliamos e quantificamos por PCR em tempo real a expressão de 9 genes envolvidos com a resposta inflamatória desencadeada pela ação da jararagina em células endoteliais humanas (HUVECs). Estes genes foram selecionados a partir de um experimento prévio de Microarray. Nossos resultados confirmaram um significante aumento na expressão da quimiocina IL-8, moléculas de adesão (E-Selectina, V-CAM-1, I-CAM-1), CD-69, Angiopoetina-2 e MMP-10. Nós também investigamos o efeito da jararagina na expressão protéica de moléculas de adesão e da quimiocina IL-8. A molécula de adesão PECAM-1 foi expressa na superfície das HUVECs em todos os tratamentos e intervalos de tempo analisados. Entretanto não observamos aumento da expressão das moléculas E-selectina e VCAM-1, na superfície de HUVECs tratadas com a jararagina. A jararagina também não induziu a liberação de IL-8 no sobrenadante de HUVECs. Uma vez que a jararagina pode se ligar a plaquetas e fibroblastos desencadeando diferentes respostas celulares nós também investigamos a interação da jararagina com as HUVECs através da integrina α 2 β 1. Nossos resultados sugerem que a jararagina se liga nas células endoteliais, mas o receptor celular envolvido neste efeito ainda não está claro. Este trabalho contribui com a literatura na medida em que elucida a participação de importantes genes dentro do contexto inflamatório desencadeado pela jararagina em células endoteliais, resultante do envenenamento botrópico. Understanding how the inflammatory response is triggered by svmps, establishes the key inflammatory mediators in the development of tissue damage and can contribute significantly to the treatment of local effects of snake envenoming. In this study we evaluated and quantified by real time PCR the expression of 9 genes involved in inflammatory response, triggered by jararhagin in human endothelial cells (HUVECS).
