Celiac disease is a common (1% prevalence) chronic immune-mediated disorder of the small intestine induced by dietary wheat, barley, and rye. Several hepatic disorders have been described in association with celiac disease. Isolated hypertransaminasemia with nonspecific histologic changes in a liver biopsy is the commonest hepatic presentation of celiac disease. A gluten-free diet normalizes liver enzymes and histologic changes in most patients. Moreover, celiac disease can coexist with autoimmune liver disorders such as autoimmune hepatitis, primary biliary cirrhosis, and primary sclerosing cholangitis. Celiac disease has increasingly been reported with a variety of other liver diseases. Thus, the hepatologist needs to consider celiac disease in the differential of abnormal liver blood tests and to be aware of the clinical implications of this frequent disease in patients with liver disorders. The possible mechanisms of liver injury and those common factors that explain the association of celiac disease with liver disorders are discussed. The aims of this article are (1) to review the spectrum and pathogenesis of liver injury related to celiac disease and (2) to provide direction to those caring for patients with chronic liver diseases regarding the detection and effective treatment of celiac disease. Concomitant occurrence of celiac disease (CeD) and Crohn's disease (CD) is rare and has been reported in several case reports and series. We report a case of a 63-year-old man with proximal CD, CeD and selective immunoglobulin (Ig) A deficiency (SIgAD). The patient was admitted in our Clinic for evaluation of epigastric pain, nausea, vomiting and weight loss (up to 7 kg) appeared in the last 3 months. The imaging findings from abdominal ultrasound and CT enterography raised suspicion of a lymphoproliferative disease of the jejunum. To exclude the development of lymphoma based on long standing untreated atypical CeD, upper endoscopy with biopsies and serological tests were conducted, showing and subtotal villous atrophy and elevated levels of IgG tissue transglutaminase tTG. Histopathology documented CD (A3,L4,B2 according to Montreal's classification).

Coeliakiában az átlagnál tízszer nagyobb a valószínűsége a gyulladásos bélbetegség kialakulásának. A fordított viszony vonatkozásában azonban jóval ellentmondásosabbak az adatok. Számos közös génpolimorfi zmus ismert, amely mindkét betegségben előfordul. Az együttes családi halmozódás, a patomechanizmusban, a szerológiai vizsgálatok-ban, a gyomor-bél rendszer makroszkópos és mikroszkópos megjelenésében mutatkozó hasonlóságok felvetik a két kórkép asszociációjának lehetőségét. A szerző 4 beteg kórtörténetét ismerteti, akik közül kettőnél Crohn-betegség és coeliakia társulása igazolódott. A másik két, gyulladásos bélbetegségben szenvedő gyermeknél szintén felmerült a coeliakia gyanúja, amelyet a kiegészítő vizsgálatok az egyik esetben megcáfoltak, a másik esetben pedig megcáfolni látszanak. A szerző hangsúlyozza, hogy a coeliakia és a gyulladásos bélbetegség egymástól való elkülönítése a klinikai gyakorlatban sokszor nem könnyű, és bármelyikük diagnosztizálása esetén gondolni kell a másik betegség esetleges jelenlétére. Kulcsszavak: coeliakia, gyulladásos bélbetegség, asszociáció Not everything is coeliac disease what it seems Concomitance of infl ammatory bowel disease and coeliac disease -Case reportThe prevalence of infl ammatory bowel disease is ten times more common in patients with celiac disease; however, studies investigating the reverse relation have contradictory fi ndings. Many gene polymorphisms are known to be present in both diseases; furthermore, similarities observed in their pathophysiological mechanism, their family concomitance, results of the serologic analysis and their macroscopic and microscopic symptoms in the gastro-intestinal system suggest a relevant association between the two diseases. The author presents the history of four patients, of whom two had both Crohn's and coeliac diseases. In the two other patients with infl ammatory bowel disease the possible diagnosis of coeliac disease was suspected, but after additional examinations coeliac disease was excluded in one patient and seemed to be unlikely in the other patient. The author concludes that the differential diagnosis of the two diseases is not easy and if one of them is diagnosed, the possible presence of the other one should be taken into consideration.
