Infective Endocarditis Complicated with Progressive Heart Failure due to β-Lactamase-Producing Cardiobacterium hominis

Abstract: We describe a 66-year-old woman with infective endocarditis due toCardiobacterium hominis whose condition, complicated by severe aortic regurgitation and congestive heart failure, necessitated aortic valve replacement despite treatment with ceftriaxone followed by ciprofloxacin. The blood isolate of C. hominis produced β-lactamase and exhibited high-level resistance to penicillin (MIC, ≧256 μg/ml) and reduced susceptibility to vancomycin (MIC, 8 μg/ml).

“…Over a quarter of patients finished treatment with β-lactam monotherapy. However, because of reports of beta-lactamaseproducing C. hominis strains causing endocarditis [24,26], it seems prudent to use a thirdgeneration cephalosporin as first-line treatment, in accordance with recently published American Heart Association guidelines [67].…”

“…Almost all isolates have been susceptible to penicillins, cephalosporins, tetracyclines, chloram-phenicol, and aminoglycosides. Recently, two isolates with beta-lactamase production have been reported [24,26]. Susceptibility testing was performed on the isolates from both of our patients.…”

“…The overall prognosis of endocarditis due to C. hominis is quite favorable, despite the frequent need for valve replacement. Penicillin along with an aminoglycoside used to be the therapy of choice and was the most frequently used regimen in the above mentioned review [8]. There have been recent reports of β-lactamase-producing isolates, with high minimal inhibitory concentration to penicillin and ampicillin [8].…”

There have been recent reports of β-lactamase-producing isolates, with high minimal inhibitory concentration to penicillin and ampicillin [8]. “…Eighty-three per cent (24 of 29) of cases in the literature have described at least one of these symptoms . Orthopnea and dyspnea may be a part of the presentation if the valvular lesion has progressed to the point where it is causing heart failure (3,38,43). This was the case with the patient described in the present report.…”
