Induction with NCB-02 (curcumin) enema for mild-to-moderate distal ulcerative colitis — A randomized, placebo-controlled, pilot study

Abstract: In this pilot study we found some evidence that use of NCB-02 enema may tend to result in greater improvements in disease activity compared to placebo in patients with mild-to-moderate distal UC. The role of NCB-02 as a novel therapy for UC should be investigated further.

“…Studies confirmed the benefits of oral curcumin administration in experimental models of chemical induced-colitis [15][16][17] . Clinical trials also confirmed the benefits of curcumin in patients with inflammatory bowel disease (IBD) and, in particular, those with ulcerative colitis 18,19 . Among the several mechanisms proposed for the action of curcumin in these patients, the antioxidant effects of the substance have pride of place 20 .…”

“…Recently, it was shown that cells of the colonic mucosa without VFAs supply suffer modifications of the cellular respiratory chain responsible for production of energy resulting in the over production of oxygen free radicals (OFR) 4 . OFR are toxic to cells and to maintain the balance between production and neutralization, all human tissues have specific antioxidant defense systems mainly our group studied the effects of curcumin in experimental models of CDC 14,[18][19][20] . In this study, we showed that the daily application of enemas containing oily extract of curcumin improved the inflammation of the colonic mucosa and decreased the neutrophil infiltrate in colonic epithelium devoid from the fecal stream…”

“…[36] Patients were evaluated using UCDAI scoring( Ulcerative Colitis Disease Activity Index) .Though the intention to treat analysis showed that treatment response was better and number of patients who had remission and had better mucosal healing were more in NCB -02 group, it was not statistically significant. But in per protocol analysis the above parameters(treatment response, remission and mucosal healing) were found to be statistically significant in NCB-02 group as compared to the control group .…”

“…But in per protocol analysis the above parameters(treatment response, remission and mucosal healing) were found to be statistically significant in NCB-02 group as compared to the control group . [36] In a randomized control trial done to study the effectiveness of curcumin on RAS (Recurrent Aphthous Stomatitis), curcumin gel was compared with triamcinolone acetonide gel and both the groups were compared with regard to reduction in the pain, number and size of the ulcers . [37] Though both the groups showed statistically significant reduction in pain, size and number of ulcers from day 0 to day 7, there was no statistical significance in the above parameters between the two groups( curcumin and triamcinolone acetonide), showing that curcumin gel is a convincing alternative to corticosteroids in the management of RAS .…”

“…A random blind study with 207 patients over 8 weeks showed curcumin to be beneficial for IBS patients, but the study did not contain a placebo control [55] . In 2 independent random double-blind placebo controlled studies, combined therapy with curcumin and mesalamine was superior over mesalamine alone [56,57] . In 2 small open studies curcumin showed marginal efficacy for IBD in adult and pediatric patients [58,59] .…”
