INDUCTION OF LABOUR WITH PROSTAGLANDIN E2

Abstract: Summary The effect of prostaglandin E2 on the activity of the pregnant human uterus in vivo has been studied in 50 women at or near term and in need of induction. Labour was successfully induced in all cases with a continuous infusion of 0·5 μg./min. The uterine activity produced by prostaglandin infusion resembled that of normal spontaneous labour. No unphysiological increase in uterine tonus was observed. The average infusion time was 5·5 hours and the average infusion‐delivery interval was 10 hours.

“…Prostaglandins (PG) of the E and F series have been known to stimulate uterine contraction in vivo (8). In the one available report of in vitro studies, PG E1 and E2 caused an increase in uterine adenylate cyclase activity, while PGF2a was inactive (7).…”

“…It was further demonstrated that contractions produced by oxytocin, acetylcholine, and calcium were inhibited by fl-adrenergic effectors, theophylline, and dibutyryl-cAMP (3, 4) in association with an increase in adenylate cyclase activity (5-7). Prostaglandins, particularly those of the E series, are potent stimulators of uterine contraction despite the fact that they stimulate uterine adenylate cyclase activity (7,8).…”

“…The uterine activity produced by prostaglandin E2 in the first and second trimesters of pregnancy is different from the prostaglandin-induced activity at or near term. On the uterus at or near term prostaglandin E2 in a concentration of 0-5-6 ,ug./minute produces normal labour-like contractions with increase in frequency and amplitude (Embrey, 1969;Karim, 1969b, Karim et al, 1970a. The tone of the uterus does not increase.…”
