Induction chemotherapy followed by concurrent radio-chemotherapy versus concurrent radio-chemotherapy alone as treatment of locally advanced squamous cell carcinoma of the head and neck (HNSCC): A meta-analysis of randomized trials

Abstract: Additional induction CHX with TPF before RT-CHX does not improve OS and PFS in locally advanced HNSCC compared to definite RT-CHX.

“…Combined modality therapy consists of surgery and radiotherapy with or without chemotherapy or molecular targeted therapy [110][111][112]. No matter what the therapeutic intervention, however, HNSCC is still associated with a high rate of recurrence [113,114].…”

“…In several phase III trials, combination of docetaxel, cisplatin (CDDP), and 5‐fluorouracil (TPF) improved clinical response and survival compared with CDDP and 5‐fluorouracil (PF) alone, and this regimen is now considered the accepted standard of care for IC . However, because no study featured CRT with CDDP in the control arm, the addition of IC (TPF) to CDDP‐based CCRT (sequential CRT) has not been shown to be superior to CDDP‐based CCRT alone . More importantly, discussion continues over whether sequential CRT can be safely administered and whether its compliance can be assured given the significant toxicities of induction TPF.…”

“…Like the aforementioned American studies, each of these international trials may be statistically underpowered, again highlighting the need for larger numbers of subjects. Even meta‐analyses that pool trials and incorporate roughly 1000 patients have demonstrated no OS improvement with induction chemotherapy …”

“…The NCDB offers one of the largest populations presently available to answer this question, including sizeable numbers with advanced disease, allowing us to overcome this limitation. Our analysis captured 2378 individuals with T4 disease and 7037 with N2b to N3 disease, comparing favorably to the 59 and 35 subjects with T4 disease and the 285 and 118 patients with N2a to N3 disease in the DeCIDE and PARADIGM trials, respectively, and to the approximately 1000 subjects with varying stages included in the meta‐analyses . Thus, even though our study of the NCDB is nonrandomized, the database offers the advantage of large numbers of subjects with advanced disease, thereby overcoming the key shortcoming of published trials and pooled analyses addressing this question.…”
