Indocyanine green and infrared fluorescence in detection of sentinel lymph nodes in endometrial and cervical cancer staging – a systematic review

Abstract: NIR fluorescence imaging using ICG is performed in robotic-assisted surgery in laparoscopy and in laparotomy, being a feasible, safe, time-efficient and seemingly reliable method for lymphatic mapping in early stage of CC and EC. Although it has promising results in SLN mapping, randomized studies, with larger patient samples, are needed.

“…It was notable that the sensitivity of ICG NIR flouresence varies according to the site of the primary malignancy. The sensitivity of ICG fluorescence ranges from 50% to 100% for endometrial and cervical carcinoma, 50‐100% for gastric cancer, 75% for esophageal carcinoma, whereas, the sensitivity in the cases of breast cancer varies from 95% to 100% with overall detection rates ranging between 88.6% and 100% …”

“…35,36 The concept of SLN was first described by Gould et al 37 39 whereas, the sensitivity in the cases of breast cancer varies from 95% to 100% 10 with overall detection rates ranging between 88.6% and 100%. 16 The present review included 12 studies that were conducted within a 10-year duration. The studies included a total of fewer than 250 patients reflecting their small sample size.…”

“…3. Cervical: The cervical injection is the modality most often reported in studies [23]. The technique is reproducible, because the uterine cervix is accessible and rarely distorted in patients with EC [24].…”

“…A single case of a severe allergic reaction after IV injection has been reported (incidence rate of 0.05%) [60]. However, the dye is contraindicated for use in patients with allergies to iodine [23,56]. Nevertheless, given that iodine is a chemical element and an essential component of the human body, a number of authors have suggested the safety of using ICG in patients with iodine allergies, given that a type I allergic reaction (antibody-mediated and responsible for anaphylactic shock) will not occur [36].…”
