BackgroundTo describe suicide rates in the indigenous and non-indigenous populations of the Nenets Autonomous Okrug (NAO) in 2002–2012, as well as associated socio-demographic characteristics.Study designRetrospective population-based mortality study.MethodsData from autopsy reports were used to identify 252 cases of suicide in the NAO in 2002–2012. Data on socio-demographic characteristics of these cases were obtained from passports and medical records at local primary health care units, and were then linked to total population data from the Censuses in 2002 and 2010. Suicide rates for the indigenous Nenets population and the non-indigenous population were standardized to the European standard population. The rates were also estimated according to different socio-demographic characteristics and compared by calculating relative risks.ResultsThe crude suicide rates were 79.8 per 100,000 person-years (PYs) in the Nenets population and 49.2 per 100,000 PYs in the non-indigenous population. The corresponding standardized estimates were 72.7 per 100,000 PYs and 50.7 per 100,000 PYs. The highest suicide rates in the Nenets population were observed in the age group 20–29 years (391 per 100,000 PYs), and in females aged 30–39 years (191 per 100,000 PYs). Socio-demographic characteristics associated with high suicide rates in the Nenets population were age 20–39 years, male, urban residence, having secondary school or higher education, being an employee or employer, and being single or divorced. Males aged 20–29 years, and females aged 30–39 and aged 70 years and above had the highest suicide rates in the non-indigenous population (137.5, 21.6 and 29.9 per 100,000 PYs, respectively). The elevated suicide rates observed in the non-indigenous population were associated with male sex, rural residence, secondary school education, being an employee or employer, and being single or divorced.ConclusionsSuicide rates in the NAO were substantially higher among the indigenous Nenets population than the non-indigenous population, and were associated with different socio-demographic characteristics.

The prevalence of APOE gene ε4/ε4 genotype and its possible contribution to the incidence of cir culatory system diseases in groups with varying degrees of "westernization" are under the consideration. It is proposed that the populations undergoing "modernization transition" with a high frequency of *ε4 carriers are in the most vulnerable position. These are Eastern Finns and especially indigenous peoples of the North, who have a higher incidence of circulatory system diseases than do residents of large cities.

ВВЕДЕНИЕПредложенная еще в 1962 году Джеймсом Нилом (Neel, 1962) гипотеза «экономного генотипа» до сих пор широко обсуждается. Выдвигаются аргу-менты за и против; примерами, в частности, могут служить дискуссия, ини-циированная редакцией International Journal of Obesity (Prentice et al., 2008; Speakman, 2008), а также недавние статьи, в которых приводятся противоре-чивые данные относительно связей между носительством потенциально «эко-номного» генотипа ε4/ε4 гена APOE и риском развития сосудистой патоло-гии (Rao et al., 2009;Llewellyn et al., 2010). Модификации подверглась даже исходная идея: в 1990-х сам автор существенно изменил свою точку зрения (Neel, 1999;Prentice et al., 2005).В наиболее общем виде гипотезу «экономного генотипа» можно предста-вить следующим образом. При нестабильности поступления питательных ве-ществ преимущество получают генотипы, способные особенно быстро утили-зировать белки, жиры или углеводы, как только те оказываются в доступности (именно поэтому такие генотипы и называют «экономными» или «бережливы-ми» -"thrifty genotypes"). При «традиционном» образе жизни такие генотипы оказываются адаптивными: риск гибели при периодических и довольно частых нехватках белка или жиров выше риска смерти от метаболических расстройств, поскольку они развиваются довольно поздно и влияют на передачу генов сле-дующему поколению меньше, чем элиминация молодых членов популяции в результате голодовки. В результате в популяции поддерживается высокая кон-центрация «экономных» генов. Однако в современных условиях «экономный генотип» повышает риск метаболических «болезней цивилизации» (таких, как атеросклероз или сахарный диабет), по ряду причин. Прежде всего, потребле-ние дефицитных ранее питательных веществ существенно возрастает из-за ста-бильной доступности и снижения стоимости. Снижение повседневного («фоно-вого») уровня физической активности приводит к тому, что питание становится избыточным не только из-за увеличения объема потребляемых нутриентов, но и вследствие дисбаланса получаемой энергии и суточных энерготрат. Наконец, в «вестернизированных» группах с их возросшей продолжительностью жизни и повысившейся долей представителей среднего и пожилого возраста (явления эпидемиологического и демографического перехода), метаболические наруше-ния не просто «успевают» аккумулироваться и на поздних стадиях онтогенеза проявляться клинически (Дильман, 1987), но и становятся одной из ведущих причин заболеваемости и смертности.С позиций медицинской антропологии можно заключить, что современный образ жизни формирует новую для Homo sapiens среду обитания, в которой отбиравшиеся на протяжении предыдущих веков и тысячелетий генетические и физиологические характеристики оказываются дезадаптивными. С эволю-ционной точки зрения, эта ситуация абсолютно нормальна: популяция посто-янно вынуждена следовать за меняющимися условиями среды обитания.
