“…This is in agreement with previous studies concerning diet-induced reduction of weight in women with PCOS (19,43) and is consistent with the findings of higher levels of T, but not D 4 A, in obese women with PCOS, compared with lean women with the syndrome (44). A possible explanation for these findings is the notion that an important portion of T in obese women is the product of D 4 A conversion in the abdominal fat (13,14,19).…”

“…IR has been proposed as a "second hit" that nonspecifically contributes to PCOS-associated follicular arrest (5,6,11). In the present study, the well-known (2,14,21) association between IR and obesity was confirmed; however, no independent significant association with PCOS was found, although a trend for higher HOMA-IR values in women with PCOS, especially its "severe" form, was observed (Tables 2 and 3). It should be noted, however, that these negative results may well have been limited by the wide spread of fasting indexes of IR values, which in small samples have been shown to be relatively inaccurate, compared with the "golden standard," i.e., the euglycemic hyperinsulinemic clamp (3).…”

“…145,152,153 Abdominal adiposity in women is usually associated with elevated circulating total and free testosterone. 154 Moreover, insulin resistance has been suggested to play an important role in the pathogenesis of PCOS.…”
