While asthma is a chronic inflammatory disorder that is managed with inhaled controller and reliever drugs, there remains a large unmet need at the severe end of the disease spectrum. Here, a novel stratified approach to its treatment is reviewed, based upon identification of causal pathways, with a focus on biologics. A systematic search of the literature was made using Medline, and publications were selected on the basis of their relevance to the topic. Despite strong preclinical data for many of the more recently identified asthma targets, especially those relating to the T-helper 2 allergic pathway, clinical trials with specific biologics in moderate to severe asthma as a group have been disappointing. However, subgroup analyses based upon pathway-specific biomarkers suggest specific endotypes that are responsive. Application of hypothesis-free analytical approaches (the 'omics') to well-defined phenotypes is leading to the stratification of asthma along causal pathways. Refinement of this approach is likely to be the future for diagnosing and treating this group of diseases, as well as helping to define new causal pathways. The identification of responders and nonresponders to targeted asthma treatments provides a new way of looking at asthma diagnosis and management, especially with biologics that are costly. The identification of novel biomarkers linked to well-phenotyped patients provides a stratified approach to disease management beyond simple disease severity and involving causal pathways. In order to achieve this effectively, a closer interaction will be required between industry (therapeutic and diagnostic), academia and health workers.

The defining feature of chronic obstructive pulmonary disease (COPD) is progressive airflow limitation that causes air trapping and hyperinflation. The increasing hyperinflation results in dyspnea along with associated inability to engage in the activities of daily living. The American Thoracic Society (ATS), European Respiratory Society (ERS) and Global Initiative for Chronic Obstructive Lung Disease (GOLD) treatment guidelines all place bronchodilators as the foundation of pharmacological management of COPD. In patients with moderate-to-very-severe respiratory impairment, adding regular treatment with one or more long-acting bronchodilators is recommended [long-acting β2-agonists (LABAs) or long-acting muscarinic antagonists (LAMAs)]. A growing body of evidence shows that LAMA and LABA co-administration is more effective than either drug class alone in managing stable COPD to improve lung function, symptoms and health status. Recently, new drug applications (NDAs) for a fixed-dose combination (FDC) of umeclidinium (UMEC), a LAMA, and vilanterol (VI), a LABA, at UMEC/VI doses of 125/25 and 62.5/25 µg have been submitted by sponsors to the US Food and Drug Administration (FDA) and to the European Medicines Agency (EMA). Thus, UMEC/VI has become the first FDC LAMA/LABA product that has reached a regulatory approval stage. Other LAMA/LABA once-daily combinations coming through development include FDCs of tiotropium and olodaterol, glycopyrronium and indacaterol, and twice-daily aclidinium and formoterol. The aim of this review is to explore currently available data for once-daily UMEC/VI in the context of the evolving standards of COPD management.

На примере 2 клинических наблюдений рассматривается воз можность перехода от монотерапии индакатеролом к комбинированной терапии индакатеролом и гликопиррония бромидом с анали зом динамики изменений соответствующих респираторных и кардиоваскулярных показателей у пациентов с ХОБЛ тяжелой степени. Продемонстрирована тенденция к уменьшению бронхообструкции, улучшению субъективных ощущений, сократительной способнос ти левого и правого желудочков и вариабельности ритма сердца. Ключевые слова: хроническая обструктивная болезнь легких, индакатерол, гликопирроний, артериальная жесткость, сократимость пра вого желудочка, вариабельность ритма сердца. SummaryThe aim of this article was to investigate respiratory and cardiovascular effects of combined therapy with indacaterol and glycopyrronium bromide in patients with severe chronic obstructive pulmonary disease. Methods. Two clinical cases are reported in the article. Both patients were treatment with long acting beta 2 agonist indacaterol for 3 months followed by 3 month treatment with combined therapy with indacaterol and glycopyrro nium bromide. We assessed arterial stiffness, pulse wave velocity rate, the heart rhythm variability, used spirometry and echocardiography, CAT and MMRC scales. Results. Combined inhalation therapy resulted in significant increase in FEV1 and FVC, reduction in dyspnea, improvement in arte rial oxygen saturation. Arterial stiffness was not changed, but the heart rhythm autonomic regulation improved in both patients. A tendency to increase in the left heart ejection fraction was also found. Conclusion. Our data are consistent with results of other clinical trials of indacaterol and glycopyrronium bromide in patients with chronic obstructive pulmonary disease.
