Increased Mortality Associated With Vitamin A Deficiency During Human Immunodeficiency Virus Type 1 Infection

Abstract: Vitamin A deficiency may be common during HIV-1 infection, and vitamin A deficiency is associated with decreased circulating CD4 T cells and increased mortality. Vitamin A is an essential micronutrient for normal immune function, and vitamin A deficiency seems to be an important risk factor for disease progression during HIV-1 infection.

“…Studies with a greater representation of drug users (>20%) consistently demonstrate deficiencies, with rates of vitamin A deficiency varying from 15–80% 610. Baum and Shor-Posner compared micronutrient deficiencies in HIV infected drug users and homosexual men.…”

“…Overall, forty one (14%) of our participants had serum vitamin A below 30 ug/dl. Low serum vitamin A has been associated with a five to six-fold increased risk of mortality in HIV infected drug users 6. However, it is not clear that micronutrient supplementation would result in an appropriate serum response in the presence of chronic HCV infection, or whether hepatic disease might block micronutrient metabolism.…”

“…Likewise, vitamin A status as an important cofactor in HIV progression has been reported. Low vitamin A concentrations were significantly associated with CD4 T-cell counts and increased progression to AIDS and as a result increased risk of mortality in HIV infected people [61, 62]. In Ethiopia, vitamin A deficiency has been reported as a severe public health problem among HIV infected patients [20, 21].…”

“…There are several possible reasons why there may be an association between the two diseases. Increased morbidity and mortality from other infectious diseases, including respiratory and diarrhoeal disease,4 measles, and HIV,5 have been associated with vitamin A deficiency 6. Both vitamin A deficiency and trachoma are associated with low socioeconomic conditions and have the highest prevalence among children between the ages of 3 and 6 years 7.…”
