Increased incidence of nonfatal cardiovascular events in stroke patients with sleep apnoea: effect of CPAP treatment

Abstract: Obstructive sleep apnoea (OSA) is a risk factor for stroke, but little is known about the effect of OSA and continuous positive airway pressure (CPAP) on the incidence of long-term, nonfatal cardiovascular events (CVE) in stroke patients.A prospective observational study was made in 223 patients consecutively admitted for stroke. A sleep study was performed on 166 of them. 31 had an apnoea/hypopnoea index (AHI) ,10 events?h -1 ; 39 had an AHI between 10 and 19 events?h -1 and 96 had an AHI o20 events?h -1 . CP… Show more

“…7,12,13,[29][30][31] NOD secondary to sleepdisordered breathing could be a modifiable risk factor for stroke and other cardiovascular diseases. 32,33 Lifestyle modification, such as weight-loss intervention, may help improve sleep-disordered breathing symptom and may possibly lead to stroke prevention. 23,34,35 In addition, long-term continuous positive airway pressure treatment, the representative treatment of sleep apnea, is considered to provide protection against stroke in patients with sleep-disordered breathing.…”

“…Study characteristics and main results for these are presented in Table 1. Six studies were European [22][23][24][25][26][27] and two were North American. 19,20 The sample sizes were small, ranging from 47 to 174 stroke or TIA patients.…”

“…19,20 The sample sizes were small, ranging from 47 to 174 stroke or TIA patients. The study designs included seven prospective cohort studies 19,20,[23][24][25][26][27] and one cross-sectional study. 22 Two studies presented stroke recurrence as a primary outcome, 22,25 five studies addressed all-cause mortality as primary outcome, 19,20,23,26,27 and one study evaluated both fatal and nonfatal cardiovascular events.…”

“…We assume that they were addressed with the patients during the frequent visits and should have been resolved by the fourth months of follow-up. The second limitation is that we did not test coping, locus of control, and self-efficacy, which have previously been shown to predict adherence [19,20,21,[26][27][28][29]. They are probably mechanisms that lead to early non-adherence that ultimately continues long-term.…”
