E mail: perisvet@list.ru Исследование посвящено изучению распространенности и структуры патологии щитовидной железы (ЩЖ) у женщин с бесплодием и оценке состояния их репродуктивной системы в зависимости от выявленной тиреоидной патологии. В основу работы положены результаты скрининговой оценки состояния ЩЖ у 496 женщин с бесплодием (основная группа) и 80 фертильных женщин (контрольная группа). Использованы традиционные методы диагностики бесплодия и специальные методы исследования, включающие оценку функции и структуры ЩЖ (уровни ТТГ, АТ ТПО, fT 4 , fT 3 , АТ рТТГ, УЗИ ЩЖ, тонкоигольная аспирационная биопсия, сцинтиграфия ЩЖ). Проведена комплексная оценка со стояния репродуктивной системы женщин с бесплодием в зависимости от выявленной патологии ЩЖ. У пациенток с бесплодием тиреоидная патология отмечена в 3,8 раза чаще, чем у фертильных женщин (48 и 12,5% соот ветственно, р < 0,05), в ее структуре преобладали носительство АТ ТПО в сочетании с ультразвуковыми признаками ауто иммунного тиреоидита (24%), гипотиреоз в исходе аутоиммунного тиреоидита (9,4%), в том числе манифестный (0,8%) и субклинический (8,6%), и эутиреоидный зоб (7,8%); доля женщин с бесплодием и тиреотоксикозом была небольшой (0,6%). Показана ассоциация аутоиммунных заболеваний ЩЖ с идиопатическим бесплодием, эндометриозом, эндо кринным бесплодием. У женщин с бесплодием необходимо проведение скрининговой оценки функции и структуры ЩЖ в рамках диагности ческого поиска причин бесплодия и своевременной коррекции выявленной тиреоидной патологии.
