Incidence, Risk Factors, and Outcomes of Retinal Detachment after Pediatric Cataract Surgery

Abstract: A 5.5% risk for RD is estimated for the first 10 years after cataract surgery in children with no known ocular and systemic anomalies. The risk significantly increases in a male, myopic, and intellectual disabled child. We emphasize the need for regular and long-term follow-up after pediatric cataract surgery.

“…One of the main challenges of postoperative pseudophakia and aphakia management is adequate postoperative visual rehabilitation that defines the further development or restoration of visual functions (Bradford et al 1994;Eriksen et al 2006). Intra-and postoperative complications related to cataract surgery, such as posterior capsular opacification, ocular hypertension, uveitis and retinal detachment, play an important role to reach best possible anatomical and functional results (Gradin & Mundia 2011;Vasavada et al 2011;Freedman et al 2015;Agarkar et al 2017). Postoperative posterior capsular opacification and visual axis re-opacification (VAR) in cases of lens-in-the-bag with or without posterior capsulorhexis belong to the most frequent complications (Tassignon et al 1996;Hosal & Biglan 2002).…”

“…Similarly, there have been limited data on the surgical outcomes of rhegmatogenous retinal detachment related to pediatric cataract surgery. In this issue of Ophthalmology, Agarkar et al 5 (see page 36) examined 481 consecutive cases of cataract surgery with primary intraocular lens implantation in 295 children with no other predisposing ophthalmic or systemic anomalies to determine the incidence and risk factors associated with postoperative retinal detachment and to study the outcomes of retinal detachment repair.…”

“…Eyes of children with intellectual disability had a 37.5% risk of retinal detachment at 10 years after cataract surgery, compared with a 3.2% risk in eyes of children with no intellectual disability. Agarkar et al 5 suggest that children with intellectual disability may be at increased risk of pseudophakic retinal detachment because of excessive rubbing and inadvertent trauma. The cumulative risk of retinal detachment at 10 years was 8.8% in eyes of children with myopia (based on ageadjusted axial length difference <À1 mm) compared with 2.1% in eyes of children with hypermetropia (ageadjusted axial length difference, >0 mm).…”
