Incidence of postoperative endophthalmitis after immediate sequential bilateral cataract surgery

Abstract: The risk for postoperative endophthalmitis in ISBCS appears to be at least as low as and possibly lower than published rates for unilateral surgery, particularly when recommended precautions are taken. Intracameral antibiotics significantly reduced the risk for postoperative endophthalmitis.

“…However, there have only been four cases of simultaneous bilateral endophthalmitis ever published [3] and all had breached the aseptic protocol published by the International Society of Bilateral Cataract Surgeons (ISBCS) [9]. In the recently reported summary of 95,606 immediate sequential bilateral cataract surgeries in America (191,212 eyes), there was no cases of bilateral endophthalmitis [13]. These data are consistent with our findings.…”

“…These technological improvements, together with the observation of very low rates of incidence of endophthalmitis, might be contributing to the observed increasing use of simultaneous bilateral cataract surgery for specific indications and selected patients 24,25. Nonetheless, we should consider that this high-frequency surgery is performed on millions of people every year globally,23,24 for which reason we need to maximize the guarantees of effectiveness and safety from the point of view of patients, and cost-effectiveness from the point of view of the health system.…”
