Incidence of permanent hypocalcaemia after total thyroidectomy with or without central neck dissection for thyroid carcinoma: a nationwide claim study

Abstract: TT plus CND was associated with a greater risk of permanent hypocalcaemia than TT alone. Surgeons should consider the risk of permanent hypocalcaemia when deciding whether to perform CND.

“…5 Most of the previous studies set the size threshold between 5 and 10 mm, but in cases when the tumor was less than this threshold, the incidence of CLNM was still high, ranging from 26% to 55%. 28,29 The results of this study showed that tumor size larger than 8 mm was independently predictive of CLNM in cN0 PTC without capsule invasion. Despite this, CLNM was present in 38.75% of cases with tumors ≤8 mm.…”

“…The role of pCND in cN0 PTC patients is controversial, with the current ATA guidelines not recommending routine pCND [ 22 ].The reason for this is that although performing pCND could detect occult CLNM, it has not been associated with improvement in recurrence or survival rates [ 23 25 ]. Additionally, patients undergoing central compartment neck dissection have been shown to suffer greater rates of morbidity, such as hypocalcemia and vocal cord paralysis, and there was also a cost-effectiveness disadvantage in comparison to patients who underwent total thyroidectomy alone [ 25 27 ]. Thus, a more selective approach is necessary to determine the requirement for pCND in patients with PTC.…”

“…When determining the surgical extent for PTC, surgical complications should be considered. Thyroidectomy has an increased a risk of hypoparathyroidism and recurrent laryngeal nerve injury compared with hemithyroidectomy . In this study, the incidence of hypoparathyroidism and vocal cord paralysis in the hemithyroidectomy group was significantly lower than those in the thyroidectomy group.…”
