In Which Patients with Sarcoidosis Is FDG PET/CT Indicated?

Abstract: Sarcoidosis is a granulomatous disease of which the etiology remains unknown. The diverse clinical manifestations may challenge clinicians, particularly when conventional markers are inconclusive. From various studies, it has become clear that fluorodeoxyglucose (FDG) positron emission tomography (PET)/CT aids in sarcoidosis care. In this article, an update on FDG PET/CT in sarcoidosis is provided. The use of FDG PET/CT in the diagnostic process of sarcoidosis is explained, especially in determining treatable … Show more

“…In case of poorly accessible organ localization without superficial nor thoracic manifestation (e.g., cardiac or neurosarcoidosis) 18 F-FDG PET may show a suggestive sarcoid-like uptake pattern with hypermetabolic mediastinal and hilar lymph nodes, whether or not combined with lung parenchymal active disease which supports the likelihood of sarcoidosis and orientates a mediastinoscopy [ 205 ]. 18 F-FDG PET may sometimes reveal smoldering superficial localizations, for example, cervical lymph nodes which are easily accessible to biopsy ( Figure 9 ) [ 206 ].…”

“…PET scan is an emerging tool for assessing inflammation in sarcoidosis as well as other inflammatory and autoimmune disorders (21,22). Uptake on PET scan, commonly defined as SUV >2.5, has been consistently associated with disease activity as well as pulmonary function in patients with sarcoidosis (23)(24)(25). The sensitivity of PET scan in identifying active inflammation ranges 89-100% and is overall higher when compared to traditional biomarkers (24).…”

“…FDG PET/CT can be especially useful in radiographic stage IV disease with fibrosis because the presence of metabolic activity indicates active disease potentially responsive to therapy. Additionally, PET/CT may show active parenchymal disease in radiographic stages 0 and 1 [ 38 , 39 ].
