Background: The paradigm shift towards the nonsurgical management of dental caries relies on the early detection of the disease. Detection of caries at an early stage is of unequivocal importance for early preventive intervention. Objective: The aim of this in vitro study is to evaluate the performance of a visual examination using the International Caries Detection and Assessment System (ICDAS) criteria, two quantitative light-induced fluorescence (QLF) systems - Inspektor™ Pro and QLF-D Biluminator™ 2 (Inspektor Research Systems B.V., Amsterdam, The Netherlands) - and a photothermal radiometry and modulated luminescence, The Canary System® (Quantum Dental Technologies, Toronto, Ont., Canada) on the detection of primary occlusal caries on permanent teeth. Methods: A total of 60 teeth with occlusal surface sites ranging from sound to noncavitated lesions (ICDAS 0-4) were assessed with each detection method twice in a random order. Histological validation was used to compare methods for sensitivity, specificity, percent correct, and the area under the receiver operating characteristic curve (AUC), at standard and optimum sound thresholds. Interexaminer agreement and intraexaminer repeatability were measured using intraclass correlation coefficients. Results: Interexaminer agreement ranged between 0.48 (The Canary System®) and 0.96 (QLF-D Biluminator™ 2). Intraexaminer repeatability ranges were 0.33-0.63 (The Canary System®) and 0.96-0.99 (QLF-D Biluminator™ 2). The sensitivity range was 0.75-0.96 while that of specificity was 0.43-0.89. The AUC were 0.79 (The Canary System®), 0.87 (ICDAS), 0.90 (Inspektor™ Pro), and 0.94 (QLF-D Biluminator™ 2). Conclusion: ICDAS had the best combination of sensitivity and specificity followed by QLF-D Biluminator™ 2 at optimum threshold.

Aim:The aim of this systematic review was to assess the in vivo scientific evidence regarding the ability of a recently developed light fluorescence device, SoproLife ® (Sopro-Acteon group, La Ciotat, France) in detecting occlusal carious lesions.The PubMed database was searched for in vivo trials that evaluated the validity of the SoproLife ® camera for the detection of occlusal carious lesions. Among the 11 articles originally identified with the keyword "Soprolife," only three articles were included.The three included surveys used the International Caries Detection and Assessment System (ICDAS)-II criteria as gold standard for the assessment of SoproLife ® compared or not to other detection devices (DIAGNOdent ® and Spectra Caries Detection Aid ® ). Two of the included studies reported only on permanent teeth or both primary and permanent teeth. The SoproLife ® validity values varied markedly among studies with a sensitivity ranging between 0.43 and 0.95 and a specificity between 0.55 and 1. Interobserver reproducibility with the SoproLife ® was reported in two of the three studies (0.98 and 0.72) and none of the studies reported about intraobserver reproducibility.No clear-cut conclusion can be made based on the three included clinical studies; further in vivo investigations are needed to confirm the validity of the SoproLife ® camera in terms of detection of occlusal carious lesions.

Abstract.Objective: The aim of this study was to evaluate the in-vivo performance of the CarieScan Pro (CS) device based on AC Impedance Spectroscopy technology (ACIST) on occlusal surfaces. Subjects and Methods: The ethics committee approved the study and the participants gave informed consent. The study included 144 unrestored permanent molars from 24 patients. The occlusal surfaces of the teeth were examined visually using ICDAS criteria. Then CarieScan measurements were performed. The actual depth of the lesions was assessed using radiographs and/or clinically by opening the lesion when appropriate. Correlation between all methods was assessed using Spearman's rank correlation coefficient (r ). Sensitivity (SE) and specificity (SP) were calculated at the D3-(dentine caries) diagnostic threshold and the area under the ROC curve (AUC) was assessed. Results: Significant positive correlation was found between ICDAS, CS measurements and the reference standard (r 0.37-0.72, P<0.0001). The AUC for ICDAS and CS were 0.93 and 0.84, respectively. Sensitivity/specificity for ICDAS and CS were 72.0%/96.6% and 68.0%/90.8%, respectively. Comparison of the ROC curves showed no significant difference between the ICDAS and CarieScan Pro performance (P = 0.129). Conclusion: Both methods showed good diagnostic performance in detection of dentine caries on occlusal surfaces.
