BACKGROUND: Birth after IVF is increasingly observed in modern obstetrics, so knowledge of its features is of paramount importance for clinical practice. AIM: The purpose of our study was to compare IVF and spontaneous singleton pregnancies in terms of cesarean section rate, operative vaginal birth rate, induction of labor, manual removal of the placenta in vaginal births, severe PPH and need for blood transfusion after delivery. MATERIAL AND METHODS: We conducted a retrospective study at "Maichin Dom" University Hospital in Sofia, Bulgaria for the period from January 2013 to December 2017 and analyzed 402 singleton IVF pregnancies and compared them with 523 spontaneous singleton pregnancies RESULTS: We found a significantly higher rate of cesarean sections in IVF compared to spontaneous pregnancies [OR 1.73; 95% CI 1.4-2.14; p<0.001]. Vaginal delivery after IVF is associated with a higher incidence of operative vaginal delivery than spontaneous pregnancies [OR 2.27; 95% CI 1.14 - 4.52; p=0.018]. No statistical differences in rates of labor induction and manual removal of the placenta between two groups were estimated (p>0.05). Severe PPH and blood transfusions were three times higher in patients after IVF undergoing a cesarean section than in those with spontaneous singleton pregnancies [OR 3.0; 95% CI 1.11 - 8.11; p=0.018]. CONCLUSION: IVF singleton pregnancies are associated with a higher risk of cesarean section as well as some obstetric complications. Obstetricians should treat these pregnancies with caution in order to reduce cesarean section rate and improve the outcome.

Backgroud: We aimed to investigate the incidence of emergency caesarean section (ECS) following problem-free frozen-thawed blastocyst transfer pregnancy.Methods: Under 39years old of 1364 nulliparous patients (including 359 cases of ART pregnancy by frozen-thawed blastocyst transfer) after 37 gestational weeks, that were planned for singleton vaginal delivery and had problem-free progress of pregnancy, were retrospectively studied; they were divided into a normal vaginal delivery group and an ECS group. We used multivariate logistic regression modeling to examine the association between the mode of conception and several factors. 122 cases of ECS were divided into two subgroups: spontaneous conception (54cases) and ART pregnancy (68cases). Surgical indications were statistically classified between groups using Fisher's exact test. Surgical findings during ECS were compared.Results: Statistical significance was observed in ART pregnancy (AOR 2.98, 95% CI 1.78-5.00, P<0.001) and infant head circumference (AOR 1.72, 95% CI 1.4-2.11, P<0.001). ECS was performed in 5.3% and 18.9% of the patients in the spontaneous conception and ART pregnancy groups, respectively. Protraction and arrest disorder were the commonest indication for ECS in both groups and was significantly higher after ART pregnancy than in spontaneous conception. There was no statistical difference between groups in the surgical findings. Conclusions:The incidence of ECS in frozen-thawed blastocyst transfer pregnancy was three times higher than that in spontaneous conception. Protraction and arrest disorder were significantly higher in ART pregnancy, although it was the most frequent indication for ECS for both groups. This information is important for patient counseling.

PurposeThe purpose of this study was to compare the sizes of the placenta and umbilical cord in women with natural pregnancy versus those undergoing in vitro fertilization (IVF).MethodsOverall, 1610 cases of uncomplicated single pregnancies with vaginal delivery at ≥ 37 weeks of gestation were included in this study. The patients were divided into two groups: natural pregnancy group (n = 1453) and IVF pregnancy not including intracytoplasmic sperm injection (ICSI) treatment (n = 157). The groups were compared in terms of gestational week, maternal age, parity, maternal weight gain, prepregnancy maternal BMI, infant weight at birth, infant head circumference, placental weight, cross section of the placenta, cross section of the umbilical cord, insertion site of the umbilical cord, and umbilical cord length. Stepwise selection and multivariate logistic regression were used for statistical analysis to correct the result as an independent factor.ResultsThere was no difference in the size of the placenta and umbilical cord between women with natural pregnancy and with IVF, but the incidence of velamentous insertion of the cord was significantly increased in women with IVF pregnancy (adjusted odd ratio [AOR] 1.72, 95% confidence interval [CI] 1.08–2.72, p = 0.026).ConclusionsAlthough there is no difference in placental weight and cord size, velamentous insertion of the umbilical cord increases in IVF pregnancy and needs careful observation during the delivery process.
