Introduction: With the increasing level of resistance to chemotherapy and radiotherapy and the associated toxicity of these therapies, the uses of plant-based compounds are gaining importance. This study was done to prove the apoptotic potential of Enicostemma axillare in breast cancer cells. Materials and Methods: 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide assay was performed involving the treatment of Michigan Cancer Foundation-7 cells for 24 h with ethanolic extract of E. axillare (EEEA) and doxorubicin to assess the cell viability, lactate dehydrogenase (LDH) was done to assess cytotoxicity, ethidium bromide (EtBr) and acridine orange (AO) assay was done to assess nuclear morphology of apoptotic cells and the expression of proteins associated with apoptosis-Bad, Bcl2, Bax, CytoC, Caspase 3, 8, and 9 were analyzed by western blotting. The data are expressed as mean±SEM. Results and Discussion: IC 50 value for EEEA is 12.5 µg/ml, whereas for doxorubicin is 1 µg/ml. A significant increased level of LDH release is seen in treated groups. EEEA and doxorubicin decrease cell viability. In AO/EtBr staining, the live cells of the EEEA treatment were similar to that of the doxorubicin. Cells stained green represent viable cells, whereas bright red staining represents late apoptotic cells. EEEA down-regulated the expression of Bcl2 (antiapoptotic protein) and up-regulated the expression of Bad and Bax (proapoptotic proteins) in MCF-7 cell line compared to control cells. In this study, EEEA treatment significantly increased the protein expression of caspase-3, 8, and 9 compared to control. Conclusions: Our finding showed that EEEA induces extrinsic and intrinsic pathway mediated apoptosis. Thus, E. axillare raises new hope for its use in breast cancer therapy.

Guaiazulene-containing ointments are routinely used for breast care during breastfeeding. It has also been proposed that nipple and areola area of non-breastfeeding women might benefit from use of this azulene. In a study with 51 women, it was investigated whether the use of a guaiazulene-containing ointment (Garmastan ® ) would improve look, elasticity and sensitivity of nipple and areola area within 4 month daily application. About one third of study participants reported improvement in sensitivity, elasticity and look of the investigated area of the breast. Assessment of doctors revealed a clear improvement in elasticity and sensitivity of the nipple and areola area over study period. Nearly all women wished to continue the use of the ointment after end of the study. In conclusion, the use of Garmastan ® ointment is able to improve sensitivity and elasticity of the nipple and areola area of the breasts of non-breastfeeding women.

Guaiazulene, which is obtained from Guaiacum officinale (Zygophyllaceae) and Matricaria chumomilla (Asteraceae), is a dark blue, crystalline bicyclic sesquiterpene. In addition to plants, it exists as a pigment in Lactarius indigo fungus and some invertebrates of the Alcyonacea group [1,2]. Guaiazulene(Gua) is a chemical with antispasmodic and antimicrobial activity. With this aspect, it is used in herbal therapy [3]. Gua, which is a cosmetic colorant approved by the FDA (Food and Drug Administration) in the USA, is used in skin -body care products, toothpaste, hair dyes, eye solutions, in the treatment of nipple irritation in nursing mothers and also to relieve itching caused by dry skin. In addition, photodynamic activation of Gua has been determined to suppress inflammation in peripheral blood mononuclear cells [4][5][6]. Azulene (AZ), a polycyclic aromatic hydrocarbon, and its derivatives, especially Gua, attract attention due to their various biological activities [7,8]. Cao et al. (2016) also determined that Gua and its derivatives show anti-gastric ulcer activity [9].
