Introduction: The aim of this systematic review is to analyse the effect of physico-chemical properties of calcium silicate-based sealers in comparison to epoxy resin sealers in permanent teeth using a single-cone obturation technique. Methods: The study was conducted according to the guidelines of Cochrane Handbook for Systematic Reviews of Interventions and Preferred Reporting Items for Systematic Review and Meta-Analyses (PRISMA) statement. Literature search was performed using the PubMed/MEDLINE, Cochrane Central Register of Controlled Trials, Web of Science, DOAJ, Open Gray with no language restrictions until October 2020. Two reviewers assessed the studies for eligibility. Grading of Recommendations, Assessment, Development, and Evaluations (GRADE) was carried out to assess the evidence. Meta-analysis of the pooled data with subgroups was performed using the RevMan software (p < 0.05). Results: Results from the 28 included studies showed that the mean difference in adaptation to root canal walls (marginal adaptation, interfacial gaps and void volume) for both sealers were non-significant. However, void volume values showed a significant mean difference (p < 0.00001) favouring the calcium silicate-based sealers. The pooled meta-analysis reported statistically significant differences for apical microleakage (p = 0.0007) whilst there were non-significant mean differences for fracture resistance (p = 0.09) and push-out bond strength (p = 0.63). The heterogeneity among the included studies was 97% (I2). Conclusions: Within the limitations of this review, calcium silicate-based sealers demonstrated a similar or superior performance in comparison to resin-based sealers in terms of the physico-chemical properties.

Background:The most desirable outcome of endodontic treatment is to achieve a fluid-tight seal of the root canal space. Root canal sealers are used in combination with core filling materials to fill the irregularities in the root canal, resulting in a three-dimensional seal that prevents bacterial regrowth. Aim: This study was aimed to compare the apical sealing ability of three root canal sealers AH Plus, Endosequence BC, and Endoseal MTA, using a single cone gutta-percha obturation technique. Materials and methods: Forty extracted human single-rooted mandibular premolar teeth were decoronated to a standardized length of 15 mm and instrumented using crown down technique with the ProTaper gold rotary file system to apical file size F3. The roots were randomly allocated into three experimental groups (n=10) and two control groups (n=5). All the samples in experimental groups were obturated with a matched taper single cone: Group 1, Group 2 and Group 3 were obturated using AH Plus, Endosequence BC and Endoseal MTA sealers, respectively. Samples were immersed in 1% Methylene blue dye solution for 72 hours, and then the roots were split longitudinally and observed under a stereomicroscope. Apical microleakage was measured from the apex to the most coronal level of dye penetration. The data obtained were subjected to statistical analysis. Results: Samples in all the groups showed evidence of leakage, except in the negative control group. One-way ANOVA showed significant differences between the groups (p=0.00132). Posthoc analysis exhibited a significant difference between group 2 and group 3 (p=0.0102). Conclusion: Endosequence BC showed a superior seal and less microleakage compared to the two other sealers used in this study using a single cone guttapercha obturation technique.

Background: Successful endodontic therapy is mainly governed by the satisfactory sealing ability of the applied root canal sealer. Also, tolerability of root canal structure to accommodate the presence of a sealer participates in the efficiency of the treatment. Hence, this study was aimed to extrapolate our previous one that was concerned with the preparation and evaluation of novel nature-based root canal sealers. Our current work is focused on the evaluation of sealing ability and in vivo biocompatibility. Materials and Methods: Egyptian propolis was extracted (ProE) and encapsulated in polymeric nanoparticles (ProE-loaded NPs). Two root sealers, PE sealer and PE nanosealer, were fabricated by incorporating ProE and ProE-loaded NPs, respectively. The sealing ability of the developed sealers was tested by a dye extraction method. An in vivo biocompatibility study was conducted using a subcutaneous implantation method for two and four weeks. At the same time, a model sealer (AH Plus®) was subjected to the same procedures to enable accurate and equitable results. Results: The teeth treated with PE sealer exhibited weak sealing ability which did not differ from that of unfilled teeth. PE nanosealer enhanced the sealing ability similarly to the model sealer with minimal apical microleakage. Studying in vivo biocompatibility indicated the capability of the three tested sealers to induce cell proliferation and tissue healing. However, PE nanosealer had superior biocompatibility, with higher potential for cell regeneration and tissue proliferation. Conclusion: PE nanosealer can be presented as an innovative root canal sealer, with enhanced sealing ability as well as in vivo biocompatibility. It can be applied as a substitute for the currently available sealers that demonstrate hazardous effects.
