ObjectiveTo describe and quantify the main changes seen on computed tomography of the chest in mildly symptomatic adult patients with sickle cell disease, as well as to evaluate the radiologist accuracy in determining the type of hemoglobinopathy.Materials and MethodsA prospective study involving 44 adult patients with sickle cell disease who underwent inspiration and expiration computed tomography of the chest. The frequency of tomography findings and the extent of involvement are reported. We also calculated radiologist accuracy in determining the type of hemoglobinopathy by analyzing the pulmonary alterations and morphology of the spleen.ResultsThe changes found on computed tomography scans, in descending order of frequency, were as follows: fibrotic opacities (81.8%); mosaic attenuation (56.8%); architectural distortion (31.8%); cardiomegaly (25.0%); lobar volume reduction (18.2%); and increased caliber of peripheral pulmonary arteries (9.1%). For most of the findings, the involvement was considered mild, five or fewer lung segments being affected. The accuracy in determining the type of hemoglobinopathy (HbSS group versus not HbSS group) was 72.7%.ConclusionIn adult patients with sickle cell disease, the main tomography findings reflect fibrotic changes. In addition, computed tomography can be helpful in differentiating among hemoglobinopathies.

Sickle-cell disease (SCD), which involves morphological changes to the red blood cells, is the most common hemoglobinopathy worldwide. This conformational change in erythrocytes affects multiple organs and systems, including the hard and soft tissues of the stomatognathic system. The objective of this study was to provide a description of the maxillomandibular positioning of patients using computed tomography in a case series of 40 patients with SCD. To define the facial profile of patients, 2-dimensional (2D) and 3-dimensional (3D) McNamara and Steiner cephalometric tracings were performed. The results showed that there is a tendency to maxillary protrusion in 2D and 3D analyses. There was no statistical difference between the 2D and 3D evaluations; additionally, sex affected the maxillomandibular positioning of patients, but only in McNamara evaluations.

Introdução: Pacientes com traço falciforme são frequentemente expostos a estresse oxidativo devido ao aumento da produção de espécies reativas derivadas do oxigênio. A doença também pode ser caracterizada pelo aumento dos níveis plasmáticos de produtos da peroxidação lipídica e do consumo de óxido nítrico. Objetivo: O objetivo do presente trabalho foi delinear o perfil epidemiológico e comparar as alterações plasmáticas dos parâmetros oxidativos (nitrito e malonaldeído (MDA)) entre portadores do traço falciforme e pacientes com perfil de hemoglobina normal. Metodologia: Foram colhidas 70 amostras, sendo 50 de portadores do traço falciforme (Grupo Hb AS) e 20 do grupo controle (Grupo Hb AA). Para a confirmação do diagnóstico laboratorial, as amostras de sangue foram submetidas ao procedimento eletroforético em pH alcalino e ao teste de solubilidade. O método empregado para determinar a peroxidação lipídica foi baseado na sua reação com ácido tiobarbitúrico e para mensurar o conteúdo de nitrito foi usado o método de GREEN. Resultados: O grupo Hb AS (0,24 ± 0,02) apresentou valor médio dos níveis séricos de MDA semelhante ao grupo Hb AA (0,20 ± 0,02; p=0,3138). Em relação aos níveis séricos de nitrito, verificou-se um aumento de 92% no grupo Hb AS (4,6 ± 0,33) quando comparado ao grupo Hb AA (2,4 ± 0,22; p<0,0009). Conclusão: Em suma, o perfil desses portadores do traço falciforme atendidos é de mulheres jovens, com baixa escolaridade, casadas e que não exercem atividade remunerada. Considerando os parâmetros oxidativos, os pacientes com traço falciforme apresentam níveis mais elevados de nitrito plasmático.
