Abstract: The substantial push for enhanced patient safety began with the publication of the National Academy of Medicine's (NAM) groundbreaking report To Err is Human: Building a Safer Health System. This report revealed that numerous patient deaths occur as the result of preventable medical errors (Kohn, Corrigan, & Donaldson, 2000).
