ABSTRACT. The objective was to identify conditions for the discontinuation of exclusive breastfeeding. This is a qualitative study developed in a Primary Health Care Unit (PHCU) in Fortaleza, Ceará, Brazil. It included 20 mothers of children aged 0-6 months old who were breastfeeding. The reports evidenced three categories: Guidance received during prenatal care; Meanings of breastfeeding; Determinants of early weaning. It can be concluded that part of the determinants of early weaning is in the building of or failure to build knowledge about breastfeeding. Cultural concepts or myths are inadequate and socially reproduced due to failure, on the part of health professionals who assist expectant mothers during prenatal care or those who assist postpartum women, to deconstruct them.Keywords: breastfeeding. weaning. social determinants of health. child health.Interfaces da descontinuidade do aleitamento materno RESUMO. Identificaremos os determinantes para a descontinuidade do aleitamento materno exclusivo. Trata-se de um estudo com abordagem qualitativa, desenvolvido em uma Unidade de Atenção Primária à Saúde (UAPS) de Fortaleza-Ceará, Brasil. Participaram 20 mães de crianças com idade de zero a seis meses em aleitamento materno. A partir dos relatos foi possível a apreensão de três categorias: orientações recebidas no pré-natal; significados da amamentação; determinantes do desmame precoce. Conclui-se que parte dos determinantes para o desmame precoce está na construção ou não construção da relevância sobre a amamentação. Os conceitos culturais ou os mitos são inadequados e seguem tendência socialmente reprodutiva, por parte dos profissionais de saúde que acompanham gestantes durante o pré-natal ou daqueles que assistem às puérperas, às construções destes.Palavras-chave: aleitamento materno. desmame. determinantes sociais da saúde. saúde da criança.

Objetivos: identificar en la literatura científica los principales factores asociados al destete temprano. Materiales y método: se trata de una revisión sistemática realizada en las bases de datos LILACS y MEDLINE con artículos completos del periodo de 2004 a 2013, en inglés, español y portugués, desde la pregunta orientadora: “¿Cuáles son los factores que influyen el destete temprano?”. Resultados: se identificaron 1.481 artículos y 39 atendieron a los criterios de inclusión. Entre los principales factores que influyen el destete temprano, se encontraron trabajo materno (33,3 %); uso de chupo de entretención (30,8 %); leche materna débil (17,9 %); trauma y dolor mamilar (17,9 %); introducción de otros tipos de leche (15,4 %) y nivel educacional de la madre o del padre (15,4 %). Conclusiones: diversos factores están relacionados al destete temprano, lo que apunta fuerte determinación sociocultural e histórica que se puede evidenciar por la comparación de los estándares de lactancia entre diferentes poblaciones y a través de los tiempos.

Objective: To analyze breastfeeding practice among indigenous children aged between zero and two years and the factors associated with ablactation. Methods: Cross-sectional study conducted with 94 indigenous children and 91 indigenous women. Data were collected in households by applying an instrument specifically developed for the study. Logistic regression was used for the analysis. Results: A total of 60.6% of the children were breastfeeding. Exclusive breastfeeding was present in 35% of the children aged under six months. The only association of early ablactation with the variables was the ethnic group, in which the chance of early ablactation among the Poyanawa, Nawa, and Nukini ethnic groups was 3.7 times higher than the Katukinas. Conclusion: The prevalence indices of breastfeeding is below the recommendations of the WHO. Only the variable ethnic group was found to be associated with early ablactation. These data highlight the need to implement programs to promote breastfeeding among indigenous people. ResumoObjetivo: Analisar o aleitamento materno de crianças indígenas de zero a dois anos e os fatores associados ao desmame. Métodos: Estudo transversal realizado com 94 crianças e 91 mulheres indígenas. Os dados foram coletados nos domicílios, com aplicação de um instrumento construído especificamente para o estudo. Para a análise foi utilizada a regressão logística. Resultados: Estavam em aleitamento materno 60,6% das crianças. Em menores de seis meses o AME esteve presente em 35% das crianças. A única associação do desmame precoce com as variáveis foi a etnia, em que a chance de desmame precoce entre as etnias Poyanawa, Nawa e Nukini, foi 3,7 vezes maior em relação a etnia Katukina. Conclusão: As prevalências de AM encontram-se aquém das recomendações da OMS. Somente a variável etnia mostrou-se associada ao desmame precoce. Esses dados mostram a necessidade de implementações de programas de incentivo ao AM entre os indígenas.
