Rheumatoid arthritis (RA) is a chronic and systemic inflammatory disease affecting 0.5–1% of the population worldwide. Interstitial lung disease (ILD) is a serious pulmonary complication of RA and it is responsible for 10–20% of mortality, with a mean survival of 5–8 years. However, nowadays there are no therapeutic recommendations for the treatment of RA-ILD. Therapeutic options for RA-ILD are complicated by the possible pulmonary toxicity of many disease modifying anti-rheumatic drugs (DMARDs) and by their unclear efficacy on pulmonary disease. Therefore, joint and lung involvement should be evaluated independently of each other for treatment purposes. On the other hand, some similarities between RA-ILD and idiopathic pulmonary fibrosis and the results of the recent INBIULD trial suggest a possible future role for antifibrotic agents. From this perspective, we review the current literature describing the pulmonary effects of drugs (immunosuppressants, conventional, biological and target synthetic DMARDs and antifibrotic agents) in patients with RA and ILD. In addition, we suggest a framework for the management of RA-ILD patients and outline a research agenda to fill the gaps in knowledge about this challenging patient cohort.

The progression of asbestosis is supposed to begin with the first order of respiratory bronchiole and extend outward. Recently, grade 4 asbestosis was reported to begin with the subpleural peripheral lobular area or the subpleural lobule. Grade 4 asbestosis is defined as diffuse pulmonary fibrosis caused by the inhalation of excessive numbers of asbestos fibers. Pathologically, the presence of more than two asbestos bodies/cm 2 on a glass slide is required. There are many cases of diffuse interstitial pneumonia, mainly usual interstitial pneumonia, that does not fulfill the above criteria among asbestos workers or high-grade environmentally exposed persons. I call these cases "usual interstitial pneumonia seen in asbestos workers" and not idiopathic pulmonary fibrosis. In this chapter, I discuss the above subjects, including the dose-response relationship for asbestos exposure, the heterogeneous response to asbestos exposure, and the relationship between asbestosis and idiopathic pulmonary fibrosis.

Ушедший в историю XX в. ознаменовался беспрецедентно активной борьбой с инфекционными заболеваниями, блестящими открытиями и достижениями в этой области. Однако в современных условиях инфекционные болезни по-прежнему сохраняют свое значение. Это объясняется динамичным характером естественных и антропогенно детерминированных природных процессов, изменением нозологической структуры инфекционных болезней, ростом международных связей и т. п. По прогнозам Всемирной организации здравоохранения, инфекционные заболевания в XXI в. вновь будут стремиться к доминирующему положению в структуре общей патологии, а также являться одной из ведущих причин смерти во всем мире. Это практически полностью относится и к ревматологии. Хорошо известно, что наличие аутоиммунного ревматического заболевания (РЗ) и необходимость применения препаратов с иммуносупрессивным действием способствуют развитию коморбидных инфекций (КИ) различной природы и локализации, существенно осложняя курацию таких пациентов. В настоящей статье будут освещены современные аспекты КИ у больных ревматоидным артритом (РА). Эпидемиология Обсуждение указанной проблемы целесообразно начать с фонового риска развития КИ, обусловленного самим наличием РА. Известно, что КИ у больных РА развиваются в 1,5 раза чаще, чем в популяции, и занимают второе-третье место среди причин смерти [1]. В ретроспективном исследовании «случай-контроль» частота КИ при РА значимо превышала таковую у здоровых: 19,64 и 12,87 на 100 пациенто-лет соответственно (относительный риск, ОР 1,53; 95% доверительный интервал, ДИ 1,41-1,65). При этом наиболее значительное повышение риска по сравнению с общей популяцией отмечалось (в порядке убывания) для септического артрита, остеомиелита, инфекций кожи и мягких тканей, пневмонии [2]. По данным многоцентрового исследования российских авторов, выполненного в начале XXI в., частота вторичных КИ у стационарного контингента больных РА составила 38,1% [3].
