ÖZETİlaçlar, fizyolojik süreçlerde güçlü ayarlayıcıdır. Optimum şartlarda kullanıldığında mortalite ve morbiditenin önemli oranda azaltılmasında etkili olmasına rağmen, ilaç hataları tıbbi hataların en yaygın nedenidir ve hastada geri dönüşü olmayan komplikasyonlar oluşabilir. İlaç kullanımı doktorun reçeteyi yazması ile başlayan, eczacı tarafından ilacın tedariki ile devam eden ve hemşire tarafından ilacın hazırlanması ve hastaya uygulanması ile son bulan multidisipliner bir süreçtir. Bu süreçlerin herhangi bir aşamasında yaşanan sorun, tıbbi hataların oluşmasının sebebidir. Bu derlemede, sağlık bilgi teknolojilerinin ilaç hatalarını engelleme ve sistemin iyileştirilmesi üzerine etkileri incelenecektir.Anahtar Kelimeler: ilaç hatası, teknoloji, hasta güvenliği SUMMARYMedications are strong regulators of physiologic processes. Despite being significant effective decreasing mortality and morbidity when used optimally, medication errors are the most common causes of medical errors and irreversible complications may occur in patients. Medication usage is a multidisciplinary process which begins with the doctor's prescription, is followed provision of medication by pharmacist and ends with the preparation and administration of the medication to the patient by nurse. The problems that exist at any stage of this process are the reason of medical errors. In this review, the effects of health information technologies on the prevention of medication errors and improvement of the system will be examined.Key words: medication error, technology, patient safety GİRİŞFarmakoloji alanında yaşanan gelişmeler tedaviyi daha etkin yönetmek ya da daha önce tedavi edilemeyen hastaları iyileştirmek amacına yönelik yeni bir takım ilaçların üretimini ve ilaç kullanımının artmasını beraberinde getirmiştir. İlaç kullanımındaki artışın bir etkisi olarak da, ilaçlara bağlı yan etkilerin, istenmeyen olayların ve ilaç hatalarının insidansında artış yaşanmıştır. Bu durum ilave tedavi gereksinimi nedeni ile hastanede kalış gün süresinin uzamasına, mortalite riskinin artmasına ve hastane maliyetlerinin yükselmesine sebep olmuştur 1 . Öte yandan ilaçlar, bir başka ifade ile modern farmakolojik ajanlar, fizyolojik süreçlerde güçlü ayarlayıcıdır 2 . Optimal kullanıldığında mortalite ve morbiditenin önemli oranda azaltılmasında etkili olmasına rağmen, ilaç hataları tıbbi hataların en yaygın nedenidir ve hastada geri dönüşü olmayan zararlar oluşturabilir. Literatürde komplikasyon ile sonuçlanan istenmeyen olayların en önemli sebeplerinden birisinin ilaç hataları olduğu, yatarak tedavi gören erişkin hastaların % 6,5'inde ve ayaktan müracaat eden erişkin hastaların % 27,4'ünde ilaç hatalarının geliştiği belirtilmektedir 3 . Barker ve arkadaşları ABD'de 36 hastanede yaptıkları çalışmada, hastaneye kabullerin en az %19'unda ilaç hatalarının geliştiğini tespit etmişlerdir 4 . Grasso ve arkadaşları ise yaptıkları çalışmada 1448 hasta gününe ilişkin hasta kayıtlarını incelemiş ve 2194 ilaç hatası geliştiğini bulmuşlardır 5 . Avustralya'da 2002-2008 yılları...

La calidad es una característica del sistema en que se entrega la atención de salud; por ende, para mejorarla, se necesita una adecuada reorganización del trabajo en equipo. Las mediciones de calidad tienen como objetivo evaluar si el proceso completo de atención de salud alcanza los objetivos deseados mientras evita los procesos que predisponen al daño del paciente. Los enfermos críticos se encuentran vulnerables frente a errores clínicos, y pueden experimentar efectos adversos prevenibles, frecuentemente asociados a medicamentos, a ventilación mec anica y a dispositivos intravasculares. La evidencia disponible actualmente indica que la seguridad y la calidad de la atención pueden ser mejoradas. En el presente artículo se exponen algunas de las estrategias e intervenciones desarrolladas para optimizar los procesos de cuidado en los pacientes críticos y mejorar la cultura de la seguridad en la unidad de cuidados intensivos. The safety and quality care are two attributes of the health care that are closely intertwined. Quality is a feature of the system that delivers health care, thereby improving it, we need a proper reorganization teamwork. Measurements of quality are intended to assess whether the process of health care reaches the desired objectives, while avoiding the processes that predispose to harm the patient. The critically ill patients are vulnerable to medical errors, and may experience side effects preventable, often associated with: medications, mechanical ventilation, and intravascular devices. The evidence currently available suggest that the safety and quality of care can be improved. In this article presents some of the strategies and interventions developed to optimize the processes of care in critically ill patients, and improve the safety culture in the ICU.

Inpatient anticoagulation services can reduce medication errors, reduce hospital costs, and improve patient care. However, before a hospital establishes an inpatient anticoagulation service, it is important to conduct a thoughtful, systematic review of the institution. Two factors that can determine the need for an inpatient anticoagulation service are the number of adverse drug events and the extent of medical-legal liability at the institution. Establishing an inpatient service that reduces these problems can justify the cost of the program. In addition to these factors, the institution's infrastructure and the scope of services to be provided should be evaluated, and parameters should be created to measure the clinical and financial impact of the inpatient anticoagulation service. Numerous publications in the literature have supported the need and positive impact of inpatient anticoagulation services on hospital costs and patient care. The size and scope of the service should be based upon the needs, experiences and resources of a specific institution.
