Impact of SARS-CoV-2 Pandemic on Colorectal Cancer Screening Delay: Effect on Stage Shift and Increased Mortality

Abstract: Background and Aims The SARS-CoV-2 pandemic had a sudden, dramatic impact on healthcare. In Italy, since the beginning of the pandemic, colorectal cancer (CRC) screening programs have been forcefully suspended. We aimed to evaluate whether screening procedure delays can affect the outcomes of CRC screening. Methods We built a procedural model considering delays in the time to colonoscopy and estimating the effect on mortality due to up-stage migration of patients. The n… Show more

“…For instance, Ricciardiello et al predicted that a 7 to 12-month delay in screening will likely result in a significant increase of stage IV cancer compared to an interval of 0 to 3 months (from 26% to 29%; p = 0.008). Deferral to more than 1 year will lead to up to 33% progression to advanced stage disease (p < 0.001) ( Ricciardiello et al, 2020 ). Vanni, in one possible scenario, estimated that a 6 months suspension of breast cancer screening will result in 16 250 temporarily missed diagnostics ( Vanni et al, 2020 ).…”

“…Insbesondere Vorsorgekoloskopien fielen in einer Größenordnung von 50 000–100 000 Untersuchungen aus oder wurden verschoben 14 15 16 . Ob diese Unterdiagnostik zu einer Übersterblichkeit oder zunehmenden Präsentation von Patienten im fortgeschrittenen Krankheitsstadium führt, bleibt abzuwarten 17 . Entsprechende Daten zu diesen Fragen können naturgemäß noch nicht vorliegen.…”

“…As shown for colorectal screening programs, delayed screening routine is likely to result in an increased proportion of advanced stage disease [ 35 ]. Thus, the continuation of widespread screening can improve patient outcomes since intervention is possible in especially early disease stages [ 35 ].…”
