Impact of Round-the-Clock, Rapid Oral Fluid HIV Testing of Women in Labor in Rural India

Abstract: BackgroundTesting pregnant women for HIV at the time of labor and delivery is the last opportunity for prevention of mother-to-child HIV transmission (PMTCT) measures, particularly in settings where women do not receive adequate antenatal care. However, HIV testing and counseling of pregnant women in labor is a challenge, especially in resource-constrained settings. In India, many rural women present for delivery without any prior antenatal care. Those who do get antenatal care are not always tested for HIV, b… Show more

“…The delivery of health care at the most peripheral levels of the health system plays an increasingly critical role in further scaling-up ART and forms a cornerstone of the UNAIDS 90-90-90 strategy. HIV viral load testing at the POC may also extend test access to settings where conventional laboratory assays are not easily accessible (11,12). POC viral load tests may also facilitate the delivery of viral load-informed differentiated care for ART patients (13).…”

“…It is often quite difficult for HIV counselors and health care providers to disclose such indeterminate results. In the context of interventions for prevention of mother-to-child transmission (PMTCT) of HIV-1 (which can require immediate decisions, notably during labor), medical staff need to know rapidly the HIV status of the woman from the laboratory in order to provide her, or not, antiretroviral (ARV) prophylaxis (5,26,27). The prolonged delay in the decision (by performing additional tests or by retesting women 14 days later) can be inappropriate in the clinical routine since women may deliver before obtaining definitive results.…”

“…The reasons behind the higher acceptability was assistance by FLWs, convenient to perform [22] and lack of fear of blood draw, or pain of needle stick etc. [9].…”

“…Multiple factors related to individual (such as inadequate knowledge on PMTCT); socio-cultural (such as stigma associated with HIV and little spouse support) and structural (such as poor transportation for HIV testing center) have been found to hinder the accessibility to HIV testing [7,8]. Another challenge for low rates of HIV testing in India is the inadequate provision of equipment essential for HIV testing including laboratory technicians and facilities [9,10]. women.…”
