Impact of reverse end-diastolic flow velocity in umbilical artery on pregnancy outcome after the 28th gestational week

Abstract: REDFV on DUAV represents an ominous and severe fetal condition with an adverse pregnancy outcome, especially in conditions associated with preeclampsia and/or intrauterine growth retardation. Intensive and frequent surveillance and aggressive management at the appropriate time would improve perinatal outcome.

“…The use of Doppler technique in obstetrics for evaluation of fetal well‐being is now generally accepted as an important clinical tool in the decision‐making, mainly in high‐risk pregnancies (1–3). To understand the physical characteristics of the blood flow velocity waveform (FVW) in the umbilical arteries, knowledge of their anatomy is important.…”

“…Abnormal Doppler velocimetry in both the uterine and umbilical arteries was significantly correlated with shorter gestational age at delivery, lower birthweight, increased cesarean section rate, and more frequent admissions to the neonatal intensive care unit [51]. Wang et al [52] recently evaluated the significance and prognosis of reversed end-diastolic flow velocity of the umbilical artery. Among 30 such patients, the mean gestational age at delivery was 31.8 ± 3.2 weeks and mean birthweight was 970 ± 270 grams.…”

“…Numerous studies have demonstrated that AED and RED flows represent an ominous sign of late fetal compromise with increased perinatal mortality and morbidity [3][4][5][6][7][8][9] . Although both AED and RED flows represent extreme pathology as regards the umbilical artery flow velocity waveform, clinical features and perinatal outcome are largely different if AED flow or RED flow is observed.…”
