Background and Design:In this study, we aimed to compare the effects of the combination of acitretin and narrow-band UVB with the combination of placebo and narrow-band UVB in the treatment of plaque-type psoriasis. Materials and Methods: A total of 50 patients with plaque-type psoriasis, in whom more than 10% of the body surface area was affected, were divided into 2 groups of 25 subjects. The first group was treated with narrow-band UVB and 25 mg/day oral acitretin and the second group was treated with narrow-band UVB and placebo. The patients were evaluated every 2 weeks during a 12 week-period. In each patient visit, psoriasis area severity index (PASI) scores were evaluated by an independent observer. Self-administered PASI (SAPASI) scores were calculated. Pre-and post-treatment quality of life was evaluated using the Skindex 29. Results:The study included a total of 50 patients (25 female, 25 male) aged 18-71 years. When PASI and SAPASI changes were evaluated, there were significant differences between the two groups at second week (p<0.05). In addition, there were significant differences between the two groups in terms of PASI and SAPASI values at 4 th , 6 th , 8 th , 10 th and 12 th weeks (p<0.001). Clinical response was found to be better in patients treated with the combination of acitretin and narrow-band UVB compared to those treated with narrow-band UVB therapy alone. Both treatment protocols were found to lead to significant improvement in the scores of the symptoms, function and emotional scales of Skindex 29. GirişPsoriasis vulgaris, etiyolojisinde genetik ve çevresel faktörlerin rol oynadığı, sık görülen, eritemli, keskin sınırlı ve skuamlı plaklar ile karakterize kronik seyirli, inflamatuvar bir deri hastalığıdır 1 . Hafif hastalıkta, topikal preparatlar psoriasis semptomlarını kontrol altına almakta genellikle yeterli olmaktadır. Orta ve şiddetli gruptaki hastalarda ise klinik yanıt alınması için fototerapi ya da sistemik ajanlara gereksinim duyulmaktadır 2 . Psoriasis hastalarında dar bant UVB geleneksel geniş bant UVB tedavisinin yerini almıştır. Yapılan karşılaştırmalı çalışmalarda dar bant UVB daha üstün bulunmuştur 3 . Dar bant UVB ve PUVA arasında yapılan karşılaştırmalı çalışmalarda hemen hemen eşit etkinlikte oldukları bildirilmiştir 4,5 . Dar bant UVB tedavisinde oral psoralen kullanılmaması, ucuz olması, hamilelerde ve çocuklarda da kullanılabilmesi, seans sonrasında gözleri korumaya gerek olmaması ve daha az karsinojen olması, dar bant UVB'yi PUVA'ya oranla daha avantajlı bir konuma getirmektedir 6 . Psoriasis tedavisinde dar bant UVB ile birlikte uygulanabilecek topikal ve sistemik tedavi yöntemleri de vardır. Sistemik tedavi kombinasyonlarından en çok retinoidler tercih edilmektedir ve tek başına dar bant UVB tedavisine göre başarı oranı daha yüksektir 7 . Oral retinoidler vitamin A deriveleri olup 1980'li yıllardan itibaren psoriasis tedavisinde kullanılmaktadır 8 . Abstract. [Purpose] To identify the relationship between depressive disorder prevalence and cognitive and physical functions of community-dwelling older adults.[Methods] The study subjects were 95 adults over the age of 65 residing in J City. Social and demographic characteristics, cognition, balance, walking, and instrumental activities of daily living (IADLs) were evaluated, along with depressive disorder symptoms, and analyzed with logistic regression.[Results] Depressive disorder among senior citizens was related to the death of a spouse, disease status, ability to walk 10 m, and IADLs. [Conclusion] Interventions that employ social and physical therapeutic approaches to enhance locomotion and activities of daily living (ADLs) through the promotion of psycholoical stability might be effective for senior citizens with depressive disorders.

Psoriasis results in expenses to patients from many cost sources. Psoriasis treatment may result in considerable time and travelling costs, yet many studies fail to account for these costs. The objective of this study was to evaluate the economic burden of psoriasis in patients. Methods: The study was based on 200 patients with psoriasis visiting a tertiary level dermatological clinic. The data was based on a patient questionnaire and clinical data from the medical records. Item costs were based on true costs charged from the patients and all time cost estimates were done. Results: 200 patients with psoriasis were included in the study. Total costs were higher for patients receiving phototherapy than those receiving systemic treatment. The majority of the visit costs arose from hospitalization and only a small proportion were attributed to outpatient visits. Conclusion: Visit charges and other patient investigations were estimated to play a minor role in the total cost of psoriasis incurred by patients, while travel costs and loss of pay comprised the majority of the costs, which should not be omitted in future studies regarding costs of treatments.
