Impact of prediabetes on poststroke depression in Chinese patients with acute ischemic stroke

Background Inflammatory cytokines are associated with the occurrence and severity of psychological disorders in cerebro‐cardiovascular disease patients. This study aimed to investigate the correlation of inflammatory cytokines with anxiety and depression in coronary heart disease (CHD) patients and their values for estimating cardiovascular outcomes. Methods Totally, 150 CHD patients and 50 healthy subjects were enrolled. Then, tumor necrosis factor (TNF)‐α, interleukin (IL)‐1β, IL‐6, IL‐10, and IL‐17 in their serum samples were detected using ELISA assay; anxiety and depression were assessed by the HADS score. For CHD patients, major adverse cardiac events (MACE) were recorded and evaluated. Results CHD patients presented with increased TNF‐α (median: 50.0 vs. 37.0 pg/ml, p < 0.001), IL‐1β (median: 2.7 vs. 2.0 pg/ml, p < 0.001), IL‐6 (median: 24.7 vs. 24.3 pg/ml, p = 0.032), IL‐17A (median: 58.6 vs. 43.6 pg/ml, p < 0.001), HADS‐A score ( p < 0.001), HADS‐D score ( p < 0.001), anxiety rate ( p < 0.001), and depression rate ( p < 0.001) compared to healthy subjects. Then, TNF‐α ( p = 0.003), IL‐1β ( p = 0.023), and IL‐17A ( p < 0.001) were related to elevated HADS‐A score. Also, TNF‐α ( p = 0.014) and IL‐17A ( p = 0.020) positively, while IL‐10 ( p = 0.047) negatively related to the HADS‐D score in CHD patients. Interestingly, elevated TNF‐α and IL‐17A were associated with anxiety and depression occurrence in CHD patients (all p < 0.05). Inspiringly, only TNF‐α high, but not other cytokines, was related to elevated accumulating MACE ( p = 0.041), while no correlation of anxiety ( p = 0.173) or depression ( p = 0.068) with accumulating MACE was observed. Conclusion TNF‐α and IL‐17A correlate with anxiety and depression, while only TNF‐α high is related to elevated accumulating MACE in CHD patients.

The early detection of major post-stroke depression (PSD) is essential to optimize patient care. A major PSD prediction tool needs to be developed and validated for early screening of major PSD patients. Patients and Methods: A total of 639 acute ischemic stroke (AIS) patients from three hospitals were consecutively recruited and completed a 3-month follow-up. Sociodemographic, clinical and laboratory test data were collected on admission. With major depression criteria being met in the DSM-V, 17-item Hamilton Rating Scale For Depression (HRSD) score ≥17 at 3 months after stroke onset was regarded as the primary endpoint. Multiple imputation was used to substitute the missing values and multivariable logistic regression model was fitted to determine associated factors with a bootstrap backward selection process. The nomogram was constructed based on the regression coefficients of the associated factors. Performance of the nomogram was assessed by discrimination (C-statistics) and calibration curve. Results: A total of 7.04% (45/639) of patients were diagnosed with major PSD at 3 months. The final logistic regression model included age, baseline NIHSS and mRS scores, educational level, calcium-phosphorus product, history of hypertension and atrial fibrillation. The model had acceptable discrimination, based on a C-statistic of 0.81 (95% CI, 0.791-0.829), with 71.1% sensitivity and 78.6% specificity. We also transformed the model to a nomogram, an easy-to-use clinical tool which could be used to facilitate the early screening of major PSD patients at 3 months. Conclusion:We identified several associated factors of major PSD at 3 months and constructed a convenient nomogram to guide follow-up and aid accurate prognostic assessment.

This paper aims to evaluate the clinical curative effect of wrist-ankle acupuncture combined with Ashi acupuncture point for the treatment of muscular strain in aerobics athletes and to provide scientific reference for future clinical treatment. 190 aerobics athletes with muscle strain treated in our hospital from March 2014 to March 2016 were randomly divided into two groups according to the random double blind method. The 95 patients in the observation group were treated with wrist-ankle acupuncture combined with Ashi acupuncture point, while the patients in the control group received body acupuncture alone. The improvement of muscle strain was compared between two groups and effective rate of treatment was evaluated. The average time of improvement in muscle strain was (3.75±1.23) d for the observation group and (6.17±1.42) d for the control group. Comparison between the two groups (p<0.05) was with significant statistical significance; the overall effective rate was 98.95 % (94/95) in the observation group and 90.53 % (86/95) in the control group. There was significant difference between the two groups (p<0.05). However, the cure rate was 81.05 % (77/95) for the observation group, but 60 % (57/95) for the control group. Comparison between the two groups (p<0.05) was with significant statistical significance. Wrist-ankle acupuncture combined with Ashi acupuncture point can effectively shorten the wound healing time and improve the cure rate of muscle strain, which has high clinical application and popularization value.

Impact of prediabetes on poststroke depression in Chinese patients with acute ischemic stroke

Abstract: Our study showed that prediabetes was associated with PSD and may predict its development at 1 month poststroke. In prediabetes subgroups, patients with HbA1c 5.7% to 6.4% were more likely to develop PSD compared to NGT and IFG/IGT groups.
