Impact of neoadjuvant chemotherapy on breast reconstruction.

Abstract: BACKGROUND-With advances in oncologic treatment, cosmesis after mastectomy has assumed a pivotal role in patient and provider decision making. Multiple studies have confirmed the safety of both chemotherapy before breast surgery and immediate reconstruction. Little has been written about the effect of neoadjuvant chemotherapy on decisions about reconstruction.

“…65 66 A retrospective review (n=665) found that patients receiving neo-adjuvant chemotherapy were less likely to undergo immediate reconstruction and more likely to undergo delayed reconstruction than those receiving adjuvant chemotherapy. 67 Evidence about whether immediate breast reconstruction delays adjuvant chemotherapy (systemic cancer directed treatment given after completion of definitive surgery and before recurrence) is conflicting; most of the studies were single institution ones, with small cohorts. 66 68-72 One large (n=3643) multicentre cohort study found that immediate breast reconstruction was associated with a modest, but significant, delay in starting treatment, particularly in patients with a high body mass index (>35).…”

“…Recent French report suggests that Skin-Sparing Mastectomy and reconstruction with a latissimus dorsi flap could be safely performed after neoadjuvant chemo-and radiotherapy, with an acceptable rate of flap necrosis in a selected group of patients (7). Patients who receive neoadjuvant chemotherapy are less likely to undergo immediate reconstruction, even though they are no more likely to undergo delayed reconstruction than patients receiving adjuvant chemotherapy (8,9). Also it has been shown that premature removal of a tissue expander occurs more commonly in patients treated with neoadjuvant or adjuvant chemotherapy and is most commonly observed 2-3 months after placement (10).…”
