Impact of Glucocorticoid Use in Oncology in the Immunotherapy Era

Abstract: Thanks to their anti-inflammatory, anti-oedema, and anti-allergy properties, glucocorticoids are among the most widely prescribed drugs in patients with cancer. The indications for glucocorticoid use are very wide and varied in the context of cancer and include the symptomatic management of cancer-related symptoms (compression, pain, oedema, altered general state) but also prevention or treatment of common side effects of anti-cancer therapies (nausea, allergies, etc.) or immune-related adverse events (irAE). … Show more

“…In this regard, the development of abnormal structural features that inhibit the ability of CTLs to penetrate tumour sites is a hallmark of cancer progression [34]. Evidence is emerging that glucocorticoids act on adhesion of immune cells by inhibiting adhesion molecules (integrins and selectins) [39,40]. The deregulation of adhesion molecules may act as barriers to T cell migration and infiltration.…”

“…Glucocorticoid has anti-inflammatory, anti-edema, anti-allergy and management cancer-related symptoms (compression, pain, edema, etc.) ( 7 ). This result argues that if hormone therapy is given at the early stage, the patient may not experience severe edema later on.…”

“…Elucidating some of the underlying mechanisms of resistance could help clinicians optimize treatment strategies. Because of the inherently pro-inflammatory therapeutic effect of ICIs, many classes of drugs that might impact the immune system may decrease their efficacy, most notably corticosteroids [ 4 ].…”
