Climate change and climate-sensitive disasters caused by climatic hazards have a significant and increasing direct and indirect impact on human health. Due to its vast area, complex geographical environment and various climatic conditions, Russia is one of the countries that suffers significantly from frequent climate hazards. This paper provides information about temperature extremities in Russia in the beginning of the 21st century, and their impact on human health. A literature search was conducted using the electronic databases Web of Science, Science Direct, Scopus, and e-Library, focusing on peer-reviewed journal articles published in English and in Russian from 2000 to 2021. The results are summarized in 16 studies, which are divided into location-based groups, including Moscow, Saint Petersburg and other large cities located in various climatic zones: in the Arctic, in Siberia and in the southern regions, in ultra-continental and monsoon climate. Heat waves in cities with a temperate continental climate lead to a significant increase in all-cause mortality than cold waves, compared with cities in other climatic zones. At the same time, in northern cities, in contrast to the southern regions and central Siberia, the influence of cold waves is more pronounced on mortality than heat waves. To adequately protect the population from the effects of temperature waves and to carry out preventive measures, it is necessary to know specific threshold values of air temperature in each city.

В последние годы интерес эпидемиологов и клиницистов привлекает вопрос влияния метеорологических факторов на соматическое и эмоциональное состояние человека. Цель-изучение метеопатических реакций у больных артериальной гипертонией (АГ) и предикторов риска их развития с использованием оригинальной анкеты. Материал и методы. Обследовано 45 больных с АГ 1-2-й степени (15 мужчин; средний возраст 57,0±11,1 года) без ишемической болезни сердца. Оценивали уровень артериального давления (АД), клинический и биохимический анализы крови, вязкость крови, скорость пульсовой волны (СПВ), показатели визуально-аналоговой шкалы качества жизни, шкалы тревоги и депрессии (HADS). Для количественной оценки выраженности метеопатических реакций разработана оригинальная анкета с 2 шкалами: Мет 1-характер и интенсивность жалоб (0-4), ассоциирующихся с метеофакторами; Мет 2-какие природные явления и в какой степени (0-3) ухудшали самочувствие. Мет = Мет 1 + Мет 2. Результаты. Чаще всего метеопатические реакции у больных АГ вызывали резкие перепады атмосферного давления и температуры (по 68,2%), магнитные бури (63,6%), жара и стабильно высокое атмосферное давление (по 54,5%). Самые частые жалобы: головные боли (54,5%), повышение АД (45,5%). Курение ассоциировалось с меньшей метеочувствительностью (Мет 38,1±18,3 у некурящих против 19,5±11,7 балла у курящих, р=0,007). Прием бета-адреноблокаторов (БАБ) ассоциировался с большей метеочувствительностью (Мет 43,4±17,0 против 28,0±17,3 балла, р=0,005). В регрессионной модели, включавшей такие факторы, как курение, тревожность, частота сердечных сокращений, прием БАБ, независимыми предикторами высокой метеочувствительности (Мет) стали: тревожность (β=0,31, р<0,05), прием БАБ (β=0,40, р<0,05), отрицательным-курение (β=-0,30, р<0,05). При введении поправки на СПВ тревожность потеряла прогностическое значение. СПВ-независимый отрицательный предиктор (β=-0,33, р<0,05) высокой Мет. Уровень натрия и АСТ-независимые предикторы высокой Мет в модели, включающей СПВ, уровень мочевой кислоты, АЛТ, АСТ, триглицеридов и натрия (β=0,63, р<0,001 и β=0,43, р<0,001 соответственно). Заключение. Метеопатические реакции наиболее часто развиваются у больных АГ 1-2-й степени при резких изменениях погодных условий, магнитных бурях, жаре, высоком атмосферном давлении. Независимые предикторы риска развития метеопатических реакций-уровень АСТ и натрия плазмы крови, отрицательные предикторы-курение, высокая СПВ.

Introduction. The increase in the incidence of cardiovascular pathology (CP) on the background of the increasing extremity of environmental factors observed in recent years requires careful analysis of the relationship between the population’s appealability of this pathology for medical assistance and factors of terrestrial and space weather in various climatic and geographical conditions. Material and methods. A retrospective analysis of the frequency of ambulance calls (AC) to CP patients, living in the foothill areas, relative to concomitant meteo- and geliogeomagnetic factors was carried out. Results. There are gender-specific, as well as seasonally specific correlations between the nosologies studied (transitory ischemic attacks (TIA), Ischemic Cardiac Disease (ICD) due to angina) and investigated factors. Significant fluctuations of these factors, their sharp inter-day fluctuations, and interaction effects contribute to an increase in the frequency of calls to the ambulance. In the dynamics of the frequency of calls, the presence of the seasonal component is traced with an emphasis on the winter period, when a spastic type of weather is formed against the background of a decrease in the average daily temperature and an increase in the atmospheric pressure, which has a negative effect on the cardiovascular system, increases risk of cardiovascular accidents. Patients with TIA have high values of systolic BP, Diastolic BP, Pulsed BP, mean BP. Against the background of “acute” meteorological conditions (IWP > 25), Diastolic BP increases, even more, the systolic and cardiac output is significantly reduced. In ICD patients, suffering from angina attacks, the maximum number of ambulance calls is recorded during the winter period due to the negative impact of both the temperature factor and the entire complex of meteorological factors in general. Attacks occurrence in the summer is minimal. Conclusion. The data obtained allows predicting the adverse effects of the terrestrial and space weather impact on the human cardiovascular system and taking preventive measures in a timely manner.
