Immunomodulatory drugs and their application to the management of canine immune‐mediated disease

Abstract: This review summarises the current understanding of immune response and T cell subsets in the context of development of autoimmunity in the dog. Mode of action and rational usage in immune‐mediated disease in the dog are discussed for the following drugs: glucocorticoids, azathioprine, cyclophosphamide, ciclosporin, tacrolimus, human intravenous immunoglobulin, vincristine, danazol, leflunomide, mycophenolate mofetil and liposome‐encapsulated clodronate. Disease mechanisms are discussed and published evidence … Show more

“…Some dogs were refractory to prednisolone and cyclosporine treatment, but responded well to leflunomide. Leflunomide, which is a relatively new immunosuppressive drug for which few reports are available in dogs [34], is a potent agent and might be a good choice in the management of uncontrollable cases, although we could not confirm this hypothesis due to the small number cases in this study. Piroxicam has been reported to be an effective agent against rectal tubulopapillary polyps in dogs [15], but was less effective than immunosuppressive therapy in the present study.…”

“…Although the efficacy of glucocorticoids in IMHA has never been investigated in a clinical trial, it is generally assumed that they are effective and their side-effects are taken for granted (Whitley and Day 2011). In contrast with recommendations for lifelong immunosuppression, the results of two studies suggest that immunosuppression for approximately 3 months is sufficient (Piek et al 2008(Piek et al , 2011.…”

“…Clearly it would be ill advised to embark upon a treatment protocol that included both steroids and non-steroidal anti-inflammatories (Whitely and Day 2011). It would therefore be useful to the clinician to have additional tools that might allow differentiation between those patients who are likely to respond to non-steroidal anti-inflammatory medication and those who may not.…”
