Diagnosis of peripheral oral exophytic lesions might be quite challenging. This review article aimed to introduce a decision tree for oral exophytic lesions according to their clinical features. General search engines and specialized databases including PubMed, PubMed Central, Medline Plus, EBSCO, Science Direct, Scopus, Embase, and authenticated textbooks were used to find relevant topics by means of keywords such as “oral soft tissue lesion,” “oral tumor like lesion,” “oral mucosal enlargement,” and “oral exophytic lesion.” Related English-language articles published since 1988 to 2016 in both medical and dental journals were appraised. Upon compilation of data, peripheral oral exophytic lesions were categorized into two major groups according to their surface texture: smooth (mesenchymal or nonsquamous epithelium-originated) and rough (squamous epithelium-originated). Lesions with smooth surface were also categorized into three subgroups according to their general frequency: reactive hyperplastic lesions/inflammatory hyperplasia, salivary gland lesions (nonneoplastic and neoplastic), and mesenchymal lesions (benign and malignant neoplasms). In addition, lesions with rough surface were summarized in six more common lesions. In total, 29 entities were organized in the form of a decision tree in order to help clinicians establish a logical diagnosis by a stepwise progression method.

• Os autores declaram que não há conflito de interesse. ResumoObjetivo: verificar se a laminina-332 é um marcador de alterações em processos iniciais da carcinogênese. Material e Métodos: trata-se de uma revisão da literatura, realizada a partir da seleção e análise de artigos originais e de revisão, publicados entre os anos de 1997 a 2013 no portal eletrônico Pubmed, Academic Google e portal de periódicos da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES). Resultados: foram selecionados 36 artigos através dos descritores: "oral"; "squamous cell carcinoma"; "laminin-332 ou laminin-5", "oral dysplastic cells" "potentially malignant disorders" que correlacionaram o avanço, progressão e/ ou invasão, do processo de progressão tumoral com a laminina-332 ou a expressão dessa glicoproteína em displasias e carcinoma oral. Conclusão: a variação da apresentação da laminina-332 apresentou relação direta com a evolução do câncer. Assim, compreender o papel dessa molécula na biologia tumoral pode auxiliar no entendimento da progressão da doença. Palavras-chave: Laminina-332 ou laminina-5; Carcinoma de células escamosas; Desordens potencialmente malignas. 1 O carcinoma oral de células escamosas (CCE) também conhecido como carcinoma espinocelular, é a neoplasia maligna de origem epitelial com maior índice de ocorrência na cavidade oral, ocupando, aproximadamente, 90-95% de todas as neoplasias nessa região. AbstrAct2 Pode desenvolver-se a partir de carcinoma in situ (CIS) ou desordens orais com potencial de malignização (DOPMs), como leucoplasia e eritroplasia, apresentando graus de diferenciação histológica. 3O CCE pode ser encontrado em qualquer sítio da boca e muitos fatores de risco estão associados ao aumento de sua incidência. O hábito de fumar e o consumo de álcool são fatores bem reconhecidos pela literatura. Isolados, esses fatores podem aumentar de duas a três vezes o risco de CCE. 4A displasia epitelial oral (DEO) é caracterizada por variação morfológica e celular nos tecidos que são similares àquelas que acontecem no CCE. São restritas ao epitélio e permanecem como uma condição não invasiva. Ela descreve um estágio pré-canceroso e sua avaliação histopatológica é um fator preditivo para a transformação maligna de DOPMs.5 Quando DOPMs apresentam a DEO e não são interceptadas precocemente, podem evoluir para CIS e até mesmo o CCE. 6 Na interface entre o epitélio e o tecido conjuntivo está a membrana basal (MB). Inicialmente, essa estrutura foi considerada como uma barreira protetora contra a invasão do câncer, com a função de impedir a invasão das células tumorais aos tecidos subjacentes. 7 No entanto, trabalhos demonstraram que a MB é uma estrutura bastante dinâmica no processo tumoral.8 Ela funciona como um complexo de proteínas ligantes e fatores de crescimento que influencia no comportamento do microambiente. Diagnostic Biomarkers in Oral Verrucous Carcinoma: A Systematic Review

Oral verrucous carcinoma (OVC), a low-grade variant of oral squamous cell carcinoma (OSCC), is most frequently seen in the oral cavity. No clear etiology has been found for this lesion, but human papilloma virus, chewing betel nuts, and ultraviolet radiation are suggested as probable causes. Differential diagnosis of OVC is challenging for oral pathologists. The aim of this study was to review the molecular-based approaches for differential diagnosis of OVC. An electronic search was conducted in Medline and Scopus from January 2004 to July 2015 limited to English language publications. Published papers on verrucous carcinoma (VC) were found according to the inclusion and exclusion criteria and analyzed qualitatively. Data extraction were performed according to PRISMA statement. A total of 423 articles were reviewed; out of which, 26 articles completely fulfilled the inclusion criteria. Most of the included studies investigated proliferative and apoptotic biomarkers such as p53 and Ki67. No definite conclusion was drawn for cytoskeletal biomarkers due to variability of factors and lack of significant expression. However, it seems that cytokeratin10 (CK 10) can be useful for differentiation of OVC and benign squamous lesions. Among cell surface and extracellular matrix biomarkers tissue biomarkers, matrix metalloproteinase (MMP)-2, -9, CD31 and CD68 seem to be useful for differentiation of OVC and OSCC and glucose transporter-1 (GLUT-1) can help in differentiation of OVC from oral epithelial dysplasia. Differences among OVC, OSCC and normal epithelium in expression profiles of the investigated biomarkers help in their differential diagnosis; although, clinicohistopathological similarities among verrucous hyperplasia, noninvasive OVC and invasive well-differentiated OSCC make the diagnosis difficult. Further studies are required to better differentiate these oral lesions.
