Immunodiagnosis of Candida-infections. I. Sensitivity of the antigens

Summary: Because of the difficulty in diagnosis of invasive candidiasis, an essay that makes use of inmunodiffusion (ID) and countedmmunoelectrophoresis (CIE) was developed to detect candida antigen in human as well as in rabbit sera. The specific antiserum was gained by immunisation of rabbits with the whole candida antigen and with the mannanprotein isolated from the cell wall of candida cell. Rabbit antiserum was tested by CIE and ID against sera from 15 cancer patients, 15 patients with respiratory disease treated for long time with corticosteroids and 15 healthy persons. Candida antigen was detected in sera from 33% of cancer patients and in 13% of asthmatics. Healthy individuals were antigen negative. Candida antibodies against the whole Candida antigen and mannanprotein were found in smaller quantities. Candida antigenemia was detected by the same methods in all rabbits given infecting doses of 108‐4 organisms/ml on the first, second and third days after the infection. The antibodies were demonstrable later and there after the antigen was no longer detectable. This sensitive and specitIc technique detects Candida antigens in the serum of infected rabbits before a widespread organ involvement occurs and should prove extremly valuable in an diagnosis of invasive candidiasis. Zusammenfassung: Die Autoren beschreiben die Möglichkeit, ein Polysaccharid‐Candida‐Antigen mittels ID und CIE in menschlichen und Kaninchenseren nachzuweisen. Das benötigte Antiserum wurde durch die Immunisierung von Kaninchen mit lebender Kultur von Candida albicans und Mannanproteinfraktion hergestellt. Das Candida‐Antigen wurde mittels ID bei 33% der Krebskranken und bei 13% der Kranken mit Respirationserkrankungen, die langfristig mit Kortikosteroiden behandelt worden waren angetroffen. Candida‐Antikörper gegen Candida albicans und der Mannanproteinfraktion wurden in niedrigerer Menge bei den Krebskranken vorgefunden. Gesunde Kontrollen waren hinsichtlich des Candida‐Antigens und der Antikörper negativ. Candidaantigenämie wurde auch bei allen infizierten Kaninchen 100% den 1., 2. und 3. Tag nach der zweiten Infektionsdosis festgestellt. Die Antikörper waren viel später nachweisbar. Die beschriebene Technik kann das Antigen gleich am Anfang der Dissemination in den Blutstrom nachweisen, wo noch keine Organveränderungen vorliegen und wann eine erfolg‐reiche Therapie eingesetzt werden kann. Candida albicans (C. albicans) produces important allergenic components which can induce allergic reactions in sensitised patients. The purpose of this study was to extract the C. albicans antigens for evaluating the specific anti-Candida IgE in sera of atopic eczema (AE) and asthmatic patients (AS). 95 AE, 85 AS, and 70 non-atopic cases were selected with sequential trials. Candida albicans antigens were prepared and then skin prick test (SPT), ELISA and IgE-immunoblotting tests were performed for all patients. Positive SPT reactions were obtained on 52.6% of AE and 54.1% of AS patients and 4.3% of healthy controls (P < 0.05). Using ELISA, specific anti-C. albicans IgE antibody was detected in 32.6% and 41.2% of patients with AE and AS, respectively. No specific IgE antibody was detected in healthy controls (P < 0.05). In SDS-PAGE, protein bands with molecular weights between 13 and 135 kDa were detected, and some of them reacted with specific IgE in immunoblotting. In AE patients, the most important allergenic components were 25, 34, and 57 kDa protein bands, whereas in AS, 22, 25, and 34 kDa protein bands were observed as major allergens. Candida albicans produces different allergenic components that can induce allergic reactions and may be pathogenetically important in patients with AE and AS.
