Background: Breastfeeding reduces incidence of common childhood illnesses such as diarrhea and thus reduces infant mortality and morbidity.Objective: To find out the incidence of diarrhea in breastfed versus formula fed baby. Methodology: An observational cross sectional study was carried out in the Pediatric department of Northern private medical college hospital, Rangpur from March 2013 to June 2014. Total 150 children aged 1 to 24 months having diarrhea were enrolled in the study.Result: Mean age of children was 11.6 with SD ±5.29 months, ranges from 1 month to 23 months in which exclusively breast fed 51.3%, breast fed plus formula fed 39.3%, exclusively formula fed 9.4%; single attack of diarrhea occurred in 72.7%, 40.7%, and 28.6% children in exclusively breast fed, breast fed plus formula fed and exclusively formula fed children respectively. Frequent attack of diarrhea occurred in 27.3%, 59.3% and 71.4% children in exclusively breast fed, breast fed plus formula fed and exclusively formula fed children respectively; among 59 patients who developed first attack of diarrhea by 6 months of age, 10.2%, 69.5 % and 20.3% were from exclusively breast fed, breast fed plus formula fed and exclusively formula fed children respectively. Patients who developed first attack of diarrhea by 7-12 months of age, 69.8% were from exclusively breast fed, 27.0% from breast fed plus formula fed and 3.2% from exclusively formula fed children. But 28 children whose first diarrhea occurred by 13-24 months of age, 96.4% and 3.6% were from exclusively breast fed, and breast fed plus formula fed group respectively.Conclusion: Breast feeding reduces incidence of diarrhea, prevents frequent attack and early occurrence of diarrhea in under two children.J Bangladesh Coll Phys Surg 2016; 34(1): 21-25

Colostrum feeding in small ruminants is crucial during the first hours after birth due to the lack of Ig transfer during pregnancy via the placenta. In addition the immature immune system of the neonate is slow to produce its own Ig during the first weeks of life. Colostrogenesis, i.e. the transfer of Ig from blood into mammary secretions, starts several weeks prepartum. In goat plasma, immunoglobulin G (IgG) concentration decreases by around 38% from the third month of gestation until partum, which coincides with the dry period. Thus, management during the dry period is crucial for the course of colostrogenesis. The colostrum synthesis is determined by the nutrition during the prepartum period, but the transfer of Ig is obviously independent of nutritional influences. The administration of conjugated linoleic acid during the dry period to dairy goats causes a less pronounced decrease of blood plasma IgG concentration (6%) but it did not change colostral IgG levels. In cattle, IgG1 is transported from blood into colostrum by an IgG1 specific receptor located on the surface of alveolar epithelial cells during colostrogenesis, and this is most likely similar in small ruminants. Via inactivation of this receptor, the Ig transfer is downregulated by increasing prolactin (PRL) during lactogenesis. It was recently observed in goats treated with PGF2a, in order to induce parturition, lower colostrum IgG concentrations occurred concomitantly with an earlier increase of plasma PRL as compared to untreated animals. The effect of litter size and number of lactations on colostral IgG concentration in small ruminants has not been made fully clear until now most likely due to the different breeds used in the published studies.

Intake of galactogogues increases breast milk secretion. Aim of this study was to investigate the Srijela (an herbal preparation) for its galactogenic property on postpartum mothers. The study was conducted on 75 postpartum mothers comprising 50 mothers who received Srijela preparation and 25 mothers as control who did not received the Srijela. The Srijela is an herbal preparation of Ashwagandha, Blackseeds, Amlaki, Methi and Hing. Observations were made on lactation factorstime of starting milk secretion after delivery, frequency of breast feeding, quantity of milk secretion, frequency of urination and weight gain of the babies. It was seen that Srijela enhanced the breast milk secretion in terms of onset, frequency, and quantity of milk secretion in the postpartum mothers as compared to those who did not received Srijela. Frequency of urination (an indicator for increase milk intake) was also remarkably higher for the babies of Srijela mothers than that for the babies of nonSrijela mothers. Weight gain of babies was also higher for Srijela mothers. This study indicated that Srijela promoted breast milk secretion in postpartum mothers.

Abstract: Colostrum has enough humoral and cellular elements to protect babies. Therefore, immune protection derived from breastfeeding depends on the immunoglobulin level of the colostrum as well as the amount of colostrum ingested.
