Immediate Placement of Implants Into Infected Sites: A Systematic Review of the Literature

Abstract: Evidence suggests implants can be placed into sites with periapical and periodontal infections. The sites must be thoroughly debrided prior to placement. Guided bone regeneration is usually performed to fill the bone-implant gap and/or socket deficiencies. Although controversial, systemic antibiotics should be used until further controlled trials prove otherwise.

“…Immediate implantation is usually avoided, however, at sites showing signs of infection, whether periodontal or endodontic 57) . Some studies have suggested that immediate implant placement should be contraindicated in the presence of infectious disease due to fear of failure of osseointegration.…”

“…Cependant, d'autres études vont plus loin et considèrent que l'implantation immédiate peut prévenir la résorption osseuse [38], tandis que d'autres auteurs ont suggéré que le placement immédiat de l'implant dans un site infecté est contre-indiqué [40,41]. …”

“…Recent comparative studies conclude that similar success rates have been reported for implants placed into infected sites compared to implants placed in non-infected sites. 1 Studies suggest that the presence of chronic infection, periapical or periodontal, does not modify osseointegration. 2 The placement of immediate implants represents an alternative to compromised teeth involved with infectious conditions.…”
