Clinical featuresBrucellosis can be subclinical, acute, subacute, and chronic. It can present itself with nonspecific symptoms such as fever, night sweats, loss of appetite, weight loss, weakness, headache, and polyarthritis (2,5-7). Brucellosis should be considered as a differential diagnosis for fevers of unknown origin. Debilitating conditions such as arthralgia, myalgia, and back pain affects around half of the patients. The incubation period is 2-3 weeks. Physical findings may vary depending on the duration of the disease. Systemic brucellosis sometimes can be complicated with meningitis, endocarditis, septic arthritis, and osteomyelitis. These complications are not extremely rare. A large meta-analysis reported the prevalence of endocarditis and neurobrucellosis as 1% and 4%, respectively (6). Osteoarticular complications are the most common complications of brucellosis (2,6,8).Relapse can occur 2-3 months after the treatment and can be detected with serologic tests and blood culture positivity. Relapse rate is common in cases treated with a single antibiotic regimen and short-term treatment. SUMMARYAlthough brucellosis is an old disease, it is still challenging for physicians. In this review, we discussed common pitfalls of brucellosis.

ÖzetBruselloz, özellikle Akdeniz ve Orta Asya ülkeleri olmak üzere dünya üzerinde yaygın dağılım gösteren, Brucella cinsindeki bakterilerle oluşan, değişik organ ve sistemleri etkileyen, sistemik infeksiyöz bir hastalıktır. İnsanlara sıklıkla infekte hayvan ürünleri, pastörize edilmemiş süt veya süt ürünlerinin tüketi-mi, infekte hayvanlarla direkt temas veya inhalasyon yoluyla bulaşır. Primer olarak retiküloendotelyal sistem hastalığı olan brusellozda, en sık (%20-60) osteoartiküler tutulum (spondilit, periferik artrit, sakroiliit ve osteomyelit) görülür. AbstractBrucellosis, prevalent especially in Mediterranean and Central Asian countries, is a systemic infectious disease which affects different organs and systems and is caused by the bacteria of the Brucella genus. It is often transmitted to humans through infected animal products, consumption of milk and milk products which are not pasteurized or direct contact with the infected animals or inhalation. Osteoarticular involvement (peripheral arthritis, spondylitis, sacroiliitis, and osteomyelitis) is often seen (20-60%) in brucellosis, which is primarily a reticuloendothelial system disease. Girişİnsan brusellozu çoğul organ tutulumu ile seyreden ve yaşamı tehdit edebilen bir hastalıktır. Brucella türleri intraselüler olarak, özellikle retiküloendotelyal sistemde (RES) yerleşmektedir (1).Bruselloz, hâlâ ciddi bir infeksiyon hastalığı olup, insan ve hayvanlarda önemli bir morbidite nedeni olmayı sürdürmektedir. Başta Akdeniz ve Orta Asya ülkeleri olmak üzere dünyanın hemen her yerinde ve her yaşta görülebilir (2). Ülkemiz brusellozun endemik olarak rapor edildiği bir ülkedir. Sağlıklı popülasyonda bruselloz seropozitifliği %2-8 arasında gözlenirken, risk grupların-da seropozitiflik %2-25 arasında değişen oranlarda göz-lenmektedir (3).Etken, Gram-negatif, sporsuz, hareketsiz, küçük kokobasil yapısında bakterilerdir. Islak ve nemli ortamlarda kolay ürerler, bu yüzden çiğ süt ve süt ürünlerinde uzun süre kalabilirler. Kaynatmakla ve pastörizasyonla inaktive olurlar (4).Tüm dünyada bruselloz vakalarında en sık izole edilen tür B. melitensis'tir. En sık rastlanan rezervuarları ise koyun ve keçilerdir. B. abortus özellikle sığırlarda bulunurken, B. suis yerel olarak görülür ve yaban domuzlarında sık olarak gözlenir. Brucellosis is a worldwide infectious disease of animals that can be transmitted to humans. Osteoarticular involvement is the most common complication of brucellosis. A 47-year-old man, who was a stockbreeder, complained of myalgia with fever and chills for 2 weeks. The serology titers and blood cultures for brucellosis were positive. Bone scintigraphy demonstrated a focally increased uptake in the left supraorbital area. Plain radiographs showed an osteolytic lesion, and an MRI revealed signal abnormalities in the corresponding site. We present an unususal case of occult Brucella osteomyelitis in the frontal bone of the skull detected by bone scintigraphy.Keywords Brucellosis . Osteomyelitis . Tc-99m MDP .
